目的：观察小剂量阿司匹林联合维生素E孕前干预D2聚体升高型复发性流产的治疗效果。方法257例血D2聚体浓度升高型复发性流产患者，2009年10月至2010年10月诊治的101例为对照组，未进行干预；2010年11月至2012年12月诊治的156例为治疗组，在妊娠前3个月口服阿司匹林50～75 mg/d、维生素E 200 mg/d ，疗程3个月。观察两组妊娠6周后血D2聚体浓度、复发性流产的发生率。结果治疗组妊娠6周时复发性流产32例（20．51％），对照组57例（56．44％），治疗组复发性流产发生率明显低于对照组（P＜0．05）；妊娠6周时，治疗组血D2聚体浓度明显低于对照组（P＜0．05）。妊娠6周时，流产组（89例）患者血D2聚体浓度明显高于未流产组（168例）（ P＜0．01）。结论阿司匹林联合维生素E孕前干预血D2聚体浓度上升型复发性流产，可有效降低复发性流产发生率，并且无严重药物不良反应。
목적：관찰소제량아사필림연합유생소E잉전간예D2취체승고형복발성유산적치료효과。방법257례혈D2취체농도승고형복발성유산환자，2009년10월지2010년10월진치적101례위대조조，미진행간예；2010년11월지2012년12월진치적156례위치료조，재임신전3개월구복아사필림50～75 mg/d、유생소E 200 mg/d ，료정3개월。관찰량조임신6주후혈D2취체농도、복발성유산적발생솔。결과치료조임신6주시복발성유산32례（20．51％），대조조57례（56．44％），치료조복발성유산발생솔명현저우대조조（P＜0．05）；임신6주시，치료조혈D2취체농도명현저우대조조（P＜0．05）。임신6주시，유산조（89례）환자혈D2취체농도명현고우미유산조（168례）（ P＜0．01）。결론아사필림연합유생소E잉전간예혈D2취체농도상승형복발성유산，가유효강저복발성유산발생솔，병차무엄중약물불량반응。
Objective To observe the therapeutic effect of low dose of aspirin combined with vitamin E on the prenatal intervention on recurrent spontaneous abortion with increased D 2 dimer.Methods Two hundred and fifty-seven recurrent spontaneous abortion patients with increased D 2 dimer were enrolled in the study ,101 cases who were treated in the hospital from October 2009 to October 2010 were in the control group ,156 cases who were treated from November 2010 to December 2012 were in the treatment group .No intervention was conducted in the control group .The patients in the treatment group orally took aspirin (50-75 mg per day) in the first three months of pregnancy as well as vitamin E (200 mg per day),and the treatment lasted for three months.The D2 dimer level and the incidence of recurrent spontaneous abortion were observed in both groups .Results There were 32 cases (20.51%) of recurrent spontaneous abortion in the treatment group and 57 cases(56.44%) in the control group within the first 6 weeks of pregnancy ,and the incidence of recurrent spontaneous abortion of treatment group was significantly lower than that of control group ( P<0 .05 ) . Within the first 6 weeks of pregnancy ,the D2 dimer level of treatment group was significantly lower than that of control group(P<0.05),which was significantly higher in the patients with recurrent spontaneous abortion (89 cases) in contrast with that in the patients without recurrent spontaneous abortion (168 cases) ( P<0.01).Conclusion The prenatal intervention on recurrent spontaneous abortion with increased D 2 dimer using aspirin and vitamin E can obviously reduce the incidence of recurrent spontaneous abortion without any serious adverse reaction of drugs .
