目的:探讨关节镜下应用自体肌腱同时重建膝关节前后交叉韧带的临床效果。方法对2008年1月至2012年12月收治的19例膝关节前后交叉韧带同时断裂的患者,切取健侧腘绳肌肌腱及双侧腓骨短肌肌腱分别对折成四股。关节镜下同时重建前后交叉韧带,股骨端应用Endobutton固定,胫骨端应用可吸收界面螺钉固定。结果19例均获随访,平均27.6个月(9~66个月)。切口Ⅰ期愈合,无感染。其中新鲜伤11例,陈旧性损伤8例。合并内侧副韧带损伤9例,予以修复。合并外侧副韧带损伤4例。术后3个月膝关节活动范围均超过120°,无伸膝受限。Lyshlom评分(90.6±3.5)分。结论关节镜下自体肌腱一期同时重建膝关节前后交叉韧带方法简便,疗效确切,能有效恢复膝关节的功能。

목적:탐토관절경하응용자체기건동시중건슬관절전후교차인대적림상효과。방법대2008년1월지2012년12월수치적19례슬관절전후교차인대동시단렬적환자,절취건측객승기기건급쌍측비골단기기건분별대절성사고。관절경하동시중건전후교차인대,고골단응용Endobutton고정,경골단응용가흡수계면라정고정。결과19례균획수방,평균27.6개월(9~66개월)。절구Ⅰ기유합,무감염。기중신선상11례,진구성손상8례。합병내측부인대손상9례,여이수복。합병외측부인대손상4례。술후3개월슬관절활동범위균초과120°,무신슬수한。Lyshlom평분(90.6±3.5)분。결론관절경하자체기건일기동시중건슬관절전후교차인대방법간편,료효학절,능유효회복슬관절적공능。
Objective To evaluate the results of an alternative procedure of arthroscopic anterrir cruciate ligment (ACL) and posterior cruciate ligment (PCL) reconstruction using hamstring tendon and peroneus brevis. Methods From January 2008 to December 2012, 19 patients with both ACL and PCL injures were treated as following:the semitendinosus, gracilis and peroneus brevis were transplanted under arthroscope and graifng were ifxed by Endobutton in fumur and absorbable interference screw in tibia. Results All patients were followed up for 9 months to 66 months, averaging 27.6 months. The incision of all patients healed without infection. There were 11 fresh and 8 old injuries. No extension limitation occurred and the lfexion degree was more than 120° in all patients 3 months postoperatively. The Lysholm knee score was (90.6±3.5). Conclusion Arthroscopic reconstruction of the ACL and PCL with hamstring tendon and peroneus brevis is effective and reliable for the restoration of knee function.
