目的：观察B超引导下经皮肝穿刺胆道引流术（PTCD）在高龄急性重症胆管炎中的治疗效果。方法：对我院收治的106例高龄急性重症胆管炎患者行PTCD，观察临床治疗效果。结果：106例均穿刺置管成功，全部获得有效胆管引流，引流24～48 h后腹痛减轻，体温逐渐降至正常。术后胆道出血2例，胆漏1例，导管脱落1例，导管堵塞2例，无气胸、血胸等并发症及死亡病例。结论：对难以耐受手术的高龄急性重症胆管炎患者，PTCD是一种安全、可靠、微创、有效的治疗方法。
목적：관찰B초인도하경피간천자담도인류술（PTCD）재고령급성중증담관염중적치료효과。방법：대아원수치적106례고령급성중증담관염환자행PTCD，관찰림상치료효과。결과：106례균천자치관성공，전부획득유효담관인류，인류24～48 h후복통감경，체온축점강지정상。술후담도출혈2례，담루1례，도관탈락1례，도관도새2례，무기흉、혈흉등병발증급사망병례。결론：대난이내수수술적고령급성중증담관염환자，PTCD시일충안전、가고、미창、유효적치료방법。
Objective:To observe the treatment effect of percutaneous transhepatic cholangio drainage (PTCD) on acute severe cholangitis in the elderly patients under guidance of B-ultrasound. Methods:In our hospital, 106 cases of elderly patients with acute severe cholangitis were treated with PTCD and their clinical treatment results were analyzed. Results:All the cases succeeded in puncture and catheterization, and got effective biliary drainage. After 24 to 48 hours all of the abdominal pain was relieved, and all of the body temperature gradually returned to normal. Among them, 2 cases were biliary tract hemorrhage, 1 case was bile leakage, catheter was fallen off in 1 case, and catheter blockage in 2 cases. There were no pneumothorax, hemothorax, other complication and no deaths. Conclusion:PTCD is a safe, reliable, minimally invasive and effective treatment method for the elderly patients with acute severe cholangitis who were dififcult to tolerate any operations.
