目的:研究完全性腭裂患者不同年龄段腭裂腭形态学定量测量统计分析,为完全性腭裂取得最佳的近期和远期效果而选择最佳的手术方案提供参考依据,并且对完全性腭裂上腭在各个年龄阶段的发育趋势进行初步探讨。方法对82例完全性腭裂患者根据手术时年龄段分成7个组,分别于手术前测量腭健侧和裂侧的直线长度、曲线长度、硬腭长、软腭长、上颌结节至裂隙距离、软腭最宽度、两侧上颌结节距离和上颌结节间裂隙宽度等指标,统计分析。结果≤12月组的上腭曲线长度健侧与裂侧的长度比较具有统计学意义,健侧长于裂侧。而在10~15周岁时,硬腭长度、上颌结节至裂隙缘长度在健侧与裂侧的比较均有统计学意义。不同年龄组的上颌结节间距与上颌结节间裂隙的宽度存在线性关系。结论不同年龄段完全性腭裂患者,上腭健侧和裂侧组织发育不一致。为完全性腭裂手术术式以及序列治疗方案,提供重要的参考依据。

목적:연구완전성악렬환자불동년령단악렬악형태학정량측량통계분석,위완전성악렬취득최가적근기화원기효과이선택최가적수술방안제공삼고의거,병차대완전성악렬상악재각개년령계단적발육추세진행초보탐토。방법대82례완전성악렬환자근거수술시년령단분성7개조,분별우수술전측량악건측화렬측적직선장도、곡선장도、경악장、연악장、상합결절지렬극거리、연악최관도、량측상합결절거리화상합결절간렬극관도등지표,통계분석。결과≤12월조적상악곡선장도건측여렬측적장도비교구유통계학의의,건측장우렬측。이재10~15주세시,경악장도、상합결절지렬극연장도재건측여렬측적비교균유통계학의의。불동년령조적상합결절간거여상합결절간렬극적관도존재선성관계。결론불동년령단완전성악렬환자,상악건측화렬측조직발육불일치。위완전성악렬수술술식이급서렬치료방안,제공중요적삼고의거。
Objective To investigate the quantitative statistical analysis of morphological of complete cleft palate in different age patients. To provide reliable reference of the best operation projects for complete cleft palate to get the best the short-term and long-term effect. Methods 82 patients with complete cleft palate were divided into 7 groups prospectively which were≤12 months,>12 months and≤24 months,>24 months and≤36 months,>36 months and≤72 months,>72 months and≤120 months(10 years),>120 months and≤180 months(15 years),>180 months(15 years). The data were measured and statistical analysed while the patient was under general anesthesia before the palatoplasty. The data were including of a straight-line and a curved distance of Palatal lengthening, the length of hard and soft palate, the distance between maxillary tuberosity and the edge of cleft, the distance between both maxillary tuberosity, the most width of one side soft palate tissue, the width of both edges of cleft on the level of maxillary tuberosity. Results It demonstrated a moderate difference in the curved distance of Palatal lengthening between the cleft side and normal side in the group of≤12 months(P=0.014). The length of normal side is longer than the cleft side. the length of hard palate, the distance between maxillary tuberosity and the edge of cleft were moderate different between the cleft side and normal side in the group of<120 months and≥180 months (0.01<P<0.05). The relationship between the distance between both maxillary tuberosity and the width of both edges of cleft on the level of maxillary tuberosity was examined to be linear in the different groups. The distance between both maxillary tuberosity(mm)= 0.99×the width of both edges of cleft on the level of maxillary tuberosity(mm)+27.177. Conclusion The quantitative statistical data of morphological of complete cleft palate in different age patients were obtained.It demonstrated the development of palatal tissue of the cleft side and normal side in different age patients were inconsonant.
