目的探讨不同手术人路治疗儿童肱骨髁上骨折的疗效.方法将确诊肱骨髁上骨折的77例患儿随机分成三组,分别通过肱三头肌纵行劈开(A组)、舌状瓣(B组)、两侧(C组)入路行手术治疗,分析并比较各组手术结果.结果 C组的闭合切口及手术时间明显少于A组和B组(P＜ 0.05和P＜0.01).C组的肘关节功能优良率明显优于A组和B组(P＜0.05).差异具有统计学意义.结论肱三头肌两侧入路手术治疗儿童肱骨髁上骨折的疗效显著.

Objective To evaluate the efficacy of three surgery approaches for the treatment of supracondylar fractures of humerus in children.Methods 77 children with supracondylar fractures of humerus were randomized into 3 groups after diagnosis.Children in Group A underwent posterior tricep splitting approach,Group B underwent lingulate flap of tricep brachii approach,and Group C underwent bilateral tricep brachii approach.The surgical results of all cases were analyzed and compared.Results The incision closure time and duration of surgery of Group C were shorter than those of Group A and B (P ＜ 0.05 and P ＜ 0.01 respectively).The excellent and good rate of elbow function evaluation in Group C was higher than those in Group A and B (P ＜ 0.05).All the differences were statistically significant.Conclusion Bilateral tricep brachii approach is an effective approach for treatment of supracondylar fracture of humerus in children.
