目的：分析比较全髋关节置换和人工股骨头置换在治疗老年人股骨颈股折的临床效果。方法将在我院治疗的135例老年股骨颈骨折患者随机分为两组，其中，人工股骨头置换组62例患者进行人工股骨头置换手术，而全髋关节置换组73例患者则进行全髋关节置换手术，分析比较两组患者的手术时间、住院时间、手术过程中的失血量、手术后的早期并发症及远期疗效等。结果全髋关节置换组的远期并发症发生率显著低于人工股骨头置换组，但在失血量和手术时间上远远高于人工股骨头置换组，而在早期并发症和住院时间等方面，两组患者之间的差距不具有显著性。结论全髋关节置换和人工股骨头置换在治疗老年人股骨颈骨折上各有优缺点，应充分考虑患者的实际情况来采用相应的手术方式。
목적：분석비교전관관절치환화인공고골두치환재치료노년인고골경고절적림상효과。방법장재아원치료적135례노년고골경골절환자수궤분위량조，기중，인공고골두치환조62례환자진행인공고골두치환수술，이전관관절치환조73례환자칙진행전관관절치환수술，분석비교량조환자적수술시간、주원시간、수술과정중적실혈량、수술후적조기병발증급원기료효등。결과전관관절치환조적원기병발증발생솔현저저우인공고골두치환조，단재실혈량화수술시간상원원고우인공고골두치환조，이재조기병발증화주원시간등방면，량조환자지간적차거불구유현저성。결론전관관절치환화인공고골두치환재치료노년인고골경골절상각유우결점，응충분고필환자적실제정황래채용상응적수술방식。
Objective To analysis the clinical effect on elderly patients with femoral neck fracture through the entire hip replacement and artiifcial femoral head replacement operation. Methods A total of 135 elderly patients with femoral neck fracture admitted to our hospital were randomly divided into two groups, and 62 cases for artiifcial femoral head replacement operation while 73 cases for entire hip replacement operation. Compare and analysis the average operation time, hospital stay, blood loss during the operation, early complications after the operation and the long term effect of the two groups. Results The incidence of long term complication in the entire hip replacement group were signiifcantly lower than those in the artiifcial femoral head replacement group, but blood loss and the average operation time were signiifcantly higher. However, the early complications and hospital stay between two groups were not signiifcant difference. Conclusion There were both advantage and disadvantage in the entire hip replacement group and artiifcial femoral head replacement group. So we should take the actual situation of the patients into full consideration and take the corresponding operation.
