目的：讨手法闭合复位探结合微创经皮接骨板内固定（MIPPO）技术治疗胫骨骨折的临床效果。方法选取本院2010年2月至2012年6月诊治的胫骨骨折患者41例，行手法闭合复位结合 MIPPO 技术治疗，术后随访1年，采用 Johner-Wruhs 关节功能评定法评价患者术后的临床疗效，采用视觉模拟评分法评定患者治疗前后的疼痛程度，分析患者的手术指征、骨折愈合情况、术后并发症情况。结果手术时间为（47.2±10.1）min，术中出血量为（58.4±15.7）mL，骨折愈合时间为（4.3±1.2）个月。术后临床疗效：优25例、良13例、可2例、差1例。优良率为92.7%。治疗后疼痛程度评分（3.1±0.9分）明显低于治疗前（5.8±1.7分），差异有统计学意义（P ＜0.05）。术后并发症发生率为4.9%。结论手法闭合复位结合 MIPPO 技术是胫骨骨折的有效治疗方法，可明显改善患者的临床病症，缓解患者的疼痛，改善患者的预后。
목적：토수법폐합복위탐결합미창경피접골판내고정（MIPPO）기술치료경골골절적림상효과。방법선취본원2010년2월지2012년6월진치적경골골절환자41례，행수법폐합복위결합 MIPPO 기술치료，술후수방1년，채용 Johner-Wruhs 관절공능평정법평개환자술후적림상료효，채용시각모의평분법평정환자치료전후적동통정도，분석환자적수술지정、골절유합정황、술후병발증정황。결과수술시간위（47.2±10.1）min，술중출혈량위（58.4±15.7）mL，골절유합시간위（4.3±1.2）개월。술후림상료효：우25례、량13례、가2례、차1례。우량솔위92.7%。치료후동통정도평분（3.1±0.9분）명현저우치료전（5.8±1.7분），차이유통계학의의（P ＜0.05）。술후병발증발생솔위4.9%。결론수법폐합복위결합 MIPPO 기술시경골골절적유효치료방법，가명현개선환자적림상병증，완해환자적동통，개선환자적예후。
Objective To investigate clinical effect of manual closed reduction and MIPPO technology in treatment for tibial fractures. Methods 41 patients with tibial fractures were selected in hospital from February 2010 to June 2012,who treated manual closed reduction and MIPPO technology. Patients were followed up for 1 year. Johner-Wruhs joint function evaluation was used to evaluate postoperative clinical effect. Visual analogue score was used to evaluate pain degree before and after treatment. Operation indication, fracture healing, postoperative complications were analyzed. Results Operation time was(47.2 ±10.1)min.Bleeding volume was 58.4±15.7ml.Fracture healing time was(4.3±1.2)months.Clinical curative effect: excellent in 25 cases, good in 13 cases, fair in 2 cases, poor in 1 case.Rate of excellent and good was 92.7%. Postoperative pain degree score(3.1±0.9)points was significantly lower than preoperative pain degree score(5.8±1.7points), while difference was statistically significant(P<0.05). Postoperative complication rate was 4.9%. Conclusion Manual closed reduction and MIPPO technology are effective method in treatment for tibial fractures,which can significantly improve clinical symptoms and relieve pain.That can also improve prognosis.
