目的：探讨钩状效应致被动凝集试验检测肺炎支原体（ mycoplasma pneumonia，MP）抗体假阴性的原因及对策，提高检验工作者对钩状效应的认识。方法按试剂盒说明书，对1例男性患儿被检血清进行倍比稀释，加致敏粒子混匀后静置3 h观察结果。结果该患儿1∶（40~160）稀释倍数反应孔为阴性，1∶（320~5120）稀释倍数反应孔为阳性，1∶（10240~20480）稀释倍数反应孔为阴性，复检后结果同前。结论钩状效应致被动凝集试验检测MP抗体假阴性少见，易发生漏诊或误诊。实验室工作人员对检测结果有疑问时，应结合患者病史及其他检查结果综合分析。
목적：탐토구상효응치피동응집시험검측폐염지원체（ mycoplasma pneumonia，MP）항체가음성적원인급대책，제고검험공작자대구상효응적인식。방법안시제합설명서，대1례남성환인피검혈청진행배비희석，가치민입자혼균후정치3 h관찰결과。결과해환인1∶（40~160）희석배수반응공위음성，1∶（320~5120）희석배수반응공위양성，1∶（10240~20480）희석배수반응공위음성，복검후결과동전。결론구상효응치피동응집시험검측MP항체가음성소견，역발생루진혹오진。실험실공작인원대검측결과유의문시，응결합환자병사급기타검사결과종합분석。
Objective To explore the causes and countermeasures of passive agglutination method in detection of false negative mycoplasma pneumonia( MP)antibody induced by hook effect so as to improve the understanding of hook effect in the clinical inspection. Methods The serum of a male child was detected by times of dilution according to the kit′s re-quirements,and then was observed having stood for 3 hours after plusing the sensitization. Results The reaction holes of 1∶(40-160)dilutions of the child were negative,the reaction holes of 1∶ (320-5120)dilution were positive,and the reaction holes of 1∶ (10240-20480)dilution were negative. The double-checked results were the same. Conclusion The passive ag-glutination method in detection of false negative MP antibody induced by hook effect is rare and may easily be missed diagnosis or misdiagnosed. Lab staff should carry out comprehensive analysis combined with the case history and other testing results if the results are suspicious.
