目的:观察免疫吸附疗法(immunoadsorption,IA)治疗神经系统免疫性疾病的临床疗效和不良反应。方法:将神经系统免疫性疾病,随机分为观察组19例和对照组19例。在常规治疗的基础上观察组采用IA治疗,对照组采用传统治疗。观察治疗前后两组患者神经功能缺损恢复情况、疗效、不良反应、免疫球蛋白IgG、IgA、IgM,补体C3、补体C4、血浆总蛋白(serum total protein,STP)含量、血细胞计数的变化。结果:两组治疗均有疗效(P>0.05)。两组治疗后(除对照组IgE外)免疫球蛋白的含量均有降低(P>0.05),观察组IgG的含量较治疗前明显降低,差异有统计学意义(P<0.01)。两组治疗前后血细胞计数无变化。结论:IA可以明显改善患者的临床症状,可能是通过降低血液中IgG的水平而起作用。

목적:관찰면역흡부요법(immunoadsorption,IA)치료신경계통면역성질병적림상료효화불량반응。방법:장신경계통면역성질병,수궤분위관찰조19례화대조조19례。재상규치료적기출상관찰조채용IA치료,대조조채용전통치료。관찰치료전후량조환자신경공능결손회복정황、료효、불량반응、면역구단백IgG、IgA、IgM,보체C3、보체C4、혈장총단백(serum total protein,STP)함량、혈세포계수적변화。결과:량조치료균유료효(P>0.05)。량조치료후(제대조조IgE외)면역구단백적함량균유강저(P>0.05),관찰조IgG적함량교치료전명현강저,차이유통계학의의(P<0.01)。량조치료전후혈세포계수무변화。결론:IA가이명현개선환자적림상증상,가능시통과강저혈액중IgG적수평이기작용。
Objective:To observe the effects and complications of immunoadsorption(IA) therapy on neurological autoimmune diseases. Methods:38 cases with neurological autoimmune diseases were divided into two groups , 19 cases were in treatment group,19 cases were in control group, 19 cases were given IA, others were given traditional therapy in addition to conventional therapy. The change of neurologic impairment, curative effect, adverse reaction, blood cell count, serum immunoglobulins of IgG, IgA, IgM, complement C3,C4 and blood plasma total protein were observed before and after therapy . Results: Clinical symptom of all patients improved after IA therapy and conventional therapy ( P>0 . 05 ) . Serum immunoglobulins of IgG decreased significantly after adsorption ( P<0 . 01 ) . Other serum immunoglobulins decreased also , except serum immunoglobulins of IgE in control group(P>0.05). Blood cell count hadn’t changed during the whole process. Conclusion:IA in treating neurological autoimmune diseases can obviously improve the symptom of patients , which may play a role by decreasing the level of IgG.
