目的:探讨复方樟柳碱联合氪激光早期治疗缺血型视网膜中央静脉阻塞(CRVO)患者血液动力学参数和中心视力的影响。方法:缺血型CRVO患者57例(57眼),随机分为治疗组29例(29眼)和对照组28例(28眼),所有患者均使用PRP治疗,治疗组另加用复方樟柳碱患眼侧颞浅动脉旁注射,14天一个疗程,连续2个疗程,治疗前后通过彩色多普勒超声检测视网膜中央动脉(CRA)的收缩期峰值血流速度(peak systolic velocity, PSV)、舒张末期血流速度(end dia-stolic velocity, EDV)和阻力指数(resistance index ,RI)及视网膜中央静脉(CRV)的最大流速(Vmax)和最小流速(Vmin),并通过比较治疗前后视力恢复的有效率来判断疗效。结果:两组在治疗后,CRA的PSV和EDV都能有效提高(P<0.05),而RI仅在复方樟柳碱治疗组有显著改善(P<0.05),复方樟柳碱治疗组可以明显改善CRV的Vmax和Vmin(P<0.05),对照组在CRV的本次研究涉及的动力学参数没有影响;复方樟柳碱治疗组在治疗后视力改善有效率达96.55%,对照组为86.71%。结论:复方樟柳碱治疗组能有效改善缺血型视网膜中央静脉阻塞的血流动力学,在改善中心视力上也具有优势。

목적:탐토복방장류감연합극격광조기치료결혈형시망막중앙정맥조새(CRVO)환자혈액동역학삼수화중심시력적영향。방법:결혈형CRVO환자57례(57안),수궤분위치료조29례(29안)화대조조28례(28안),소유환자균사용PRP치료,치료조령가용복방장류감환안측섭천동맥방주사,14천일개료정,련속2개료정,치료전후통과채색다보륵초성검측시망막중앙동맥(CRA)적수축기봉치혈류속도(peak systolic velocity, PSV)、서장말기혈류속도(end dia-stolic velocity, EDV)화조력지수(resistance index ,RI)급시망막중앙정맥(CRV)적최대류속(Vmax)화최소류속(Vmin),병통과비교치료전후시력회복적유효솔래판단료효。결과:량조재치료후,CRA적PSV화EDV도능유효제고(P<0.05),이RI부재복방장류감치료조유현저개선(P<0.05),복방장류감치료조가이명현개선CRV적Vmax화Vmin(P<0.05),대조조재CRV적본차연구섭급적동역학삼수몰유영향;복방장류감치료조재치료후시력개선유효솔체96.55%,대조조위86.71%。결론:복방장류감치료조능유효개선결혈형시망막중앙정맥조새적혈류동역학,재개선중심시력상야구유우세。
Purpose: To explore the combined effect of compound anisodine and pan-retinal photocoagulation (PRP) on the he-modynamic and central vision in patients with ischemic central retinal vein occlusion (CRVO).Methods: 57 cases 57 eyes with CRVO were enrolled in the study. They were randomly divided into treatment group (29 cases 29 eyes) and control group (28 cases 28 eyes). All the patients were received standard PRP. The patients in treatment group were injected compound anisondine around superficial temporal artery additionally. The central vision acuity and hemodynamic parameters measured by color Doppler flow imaging (CDFL) were observed before and after treatment.Results:After treatment, PSV, EDV, RI, Vmax and Vmin were im-proved significantly in treatment group, only PSV, EDV were improved significantly in control group. After 2 months follow up, the central visual acuity of 28 improved (efficiency 96.55%) in treatment group, while which of 24 improved (efficiency86.71%) in control group.Conclusions: compound anisodine and PRP can improve the hemodynamic parameters and central vision acuity in patients with ischemic CRVO.
