目的:探讨早产儿预防性使用固尔苏的效果。方法:采用试验研究的方法,选取本院产科出生、生后极早期(30 min)内预防性使用固尔苏的87例早产儿为观察组,由外院转诊至本院新生儿科,生后6 h内预防性使用固尔苏的36例早产儿为对照组,所有早产儿胎龄均≤32周。比较两组早产儿一般情况、用药前和用药后不同时段的氧分压、二氧化碳分压指标及NRDS的转归等情况。结果:观察组使用固尔苏后1、6、12 h的氧分压高于对照组、二氧化碳分压低于对照组,差异均有统计学意义(P<0.05);观察组NRDS的发生率为20%,显著低于对照组的60%(P<0.05);观察组机械通气例数少于对照组,但差异无统计学意义(P>0.05)。结论:极早期预防性使用固尔苏效果好,可有效改善肺通气情况和氧合能力,降低NRDS的发生率,减少机械通气例数。

목적:탐토조산인예방성사용고이소적효과。방법:채용시험연구적방법,선취본원산과출생、생후겁조기(30 min)내예방성사용고이소적87례조산인위관찰조,유외원전진지본원신생인과,생후6 h내예방성사용고이소적36례조산인위대조조,소유조산인태령균≤32주。비교량조조산인일반정황、용약전화용약후불동시단적양분압、이양화탄분압지표급NRDS적전귀등정황。결과:관찰조사용고이소후1、6、12 h적양분압고우대조조、이양화탄분압저우대조조,차이균유통계학의의(P<0.05);관찰조NRDS적발생솔위20%,현저저우대조조적60%(P<0.05);관찰조궤계통기례수소우대조조,단차이무통계학의의(P>0.05)。결론:겁조기예방성사용고이소효과호,가유효개선폐통기정황화양합능력,강저NRDS적발생솔,감소궤계통기례수。
Objective:To discuss the effect of premature infants preventive use curosurf. Method:Adopted the method of experimental research. Selected 87 cases of premature infants that were born or postnatal 30 minutes to use curosurf preventively as observation group. And selected 36 cases of premature infants that were sent to our hospital from other hospitals as control group to use curosurf preventively within 6 hours. All premature infants’ gestational age were greater than or equal to 32 weeks. Investigated the two groups of premature babies and survey premature medication at different times of the oxygen partial pressure,carbon dioxide,and blood samples before and after saturation and other indexes and conversion of NRDS. Result:PaO2 of the observation group with curosurf after 1,6,12 h was higher than the control group,PaCO2 was lower than the control group,the difference had statistical significance(P<0.05);the incidence of NRDS in the observation group was 20%,and significantly less than 60%in the control group(P<0.05);mechanical ventilation cases of the observation group was less than the control group,but there was no statistically significant difference(P>0.05). Conclusion:Prophylactic use curosurf effect is good. It can effectively improve pulmonary ventilation and oxygenation capacity,reduce the incidence of NRDS,reduce the mechanical ventilation cases.
