目的：探讨常用的肿瘤标志物癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原19-9(CA19-9)、糖类抗原15-3(CA15-3)在乳腺癌诊断及疗效评价中的应用价值。方法采用电化学发光法检测102例乳腺癌患者、38例乳腺良性肿瘤患者、78例健康人血清中4种肿瘤标志物的水平。结果 CEA、CA125、CA15-3、CA19-9等4种标志物在乳腺癌患者血清中的含量均高于乳腺良性肿瘤患者和健康人，具有统计学意义( P<0.05)；4种标志物对乳腺癌诊断的敏感性随着肿瘤分期的加重而逐渐提高，但是仅CEA和CA15-3诊断乳腺癌的阳性率相对较高；联合检测CEA+ CA15-3诊断乳腺癌的性价比最高；乳腺癌经过手术或化疗后血清CEA、CA15-3、CA125下降明显，CA19-9下降不明显。结论血清肿瘤标志物尤其是CEA和CA15-3对于乳腺癌的诊断和监测具有良好的临床应用价值。
목적：탐토상용적종류표지물암배항원(CEA)、당류항원125(CA125)、당류항원19-9(CA19-9)、당류항원15-3(CA15-3)재유선암진단급료효평개중적응용개치。방법채용전화학발광법검측102례유선암환자、38례유선량성종류환자、78례건강인혈청중4충종류표지물적수평。결과 CEA、CA125、CA15-3、CA19-9등4충표지물재유선암환자혈청중적함량균고우유선량성종류환자화건강인，구유통계학의의( P<0.05)；4충표지물대유선암진단적민감성수착종류분기적가중이축점제고，단시부CEA화CA15-3진단유선암적양성솔상대교고；연합검측CEA+ CA15-3진단유선암적성개비최고；유선암경과수술혹화료후혈청CEA、CA15-3、CA125하강명현，CA19-9하강불명현。결론혈청종류표지물우기시CEA화CA15-3대우유선암적진단화감측구유량호적림상응용개치。
Objective To evaluate the clinical value of CEA、CA125、CA15-3 and CA19-9 used alone or in combination for the diagnosis and therapeutic evaluation of breast cancer. Methods The serum levels of four tumor markers:CEA、CA125、CA15-3 and CA19-9 were detected by Elecsys 2010 electrochemiluminescence analysator in 102 patients with breast cancer,38 patients with breast benign tumor and 78 healthy adults.Results The serum levels of the four tumor markers were significantly higher in breast cancer patients than in breast benign tumor patients and healthy adults( p<0.05). We revealed an increasing tendency in serum tumor marker levels, along with the worsening of the breast cancer stages. Conclusion These results indicate that the serum tumor markers, especial y serum CEA and CA15-3 have good clinical values for the diagnosis and surveil ance of breast cancer.
