目的:观察益气渗湿中药汤剂对预防髋关节置换术后下肢深静脉血栓形成的影响。方法将63例患者按随机对照原则分为A组和B组,手术术式为全麻下行人工髋关节置换术,术后A组给予低分子肝素钙,B组在A组基础上给予益气渗湿类中药汤剂。给药1周,观察下肢股静脉血流速度、彩超是否有下肢深静脉血栓形成、血小板计数及纤维蛋白原数值、疼痛评分(VAS)及肢体周径的变化,并比较组间差异。结果2组下肢股静脉血流速度、血小板计数及纤维蛋白原数值术后3,7 d各时段组间比较,差异均有统计学意义(P<0.05);2组VAS评分术后3 d组间比较,差异有统计学意义(P<0.05)。结论益气渗湿法可有效地降低髋关节置换术后下肢静脉血栓发生的风险。

목적:관찰익기삼습중약탕제대예방관관절치환술후하지심정맥혈전형성적영향。방법장63례환자안수궤대조원칙분위A조화B조,수술술식위전마하행인공관관절치환술,술후A조급여저분자간소개,B조재A조기출상급여익기삼습류중약탕제。급약1주,관찰하지고정맥혈류속도、채초시부유하지심정맥혈전형성、혈소판계수급섬유단백원수치、동통평분(VAS)급지체주경적변화,병비교조간차이。결과2조하지고정맥혈류속도、혈소판계수급섬유단백원수치술후3,7 d각시단조간비교,차이균유통계학의의(P<0.05);2조VAS평분술후3 d조간비교,차이유통계학의의(P<0.05)。결론익기삼습법가유효지강저관관절치환술후하지정맥혈전발생적풍험。
Objective To observe the preventive effect of decoction with the actions of tonifying qi and draining dampness on lower extremity deep vein thrombosis (DVT) in the patients after hip arthroplasty. Method The patients (n=63) were randomly divided into basic treatment group (group A) and basic treatment combined with decoction group (group B). All the patients were operated by general anesthesia. The postoperative patients in group A were administrated with low molecular weight heparin (LMWH), while the patients in group B were additionally given oral decoction with actions of tonifying qi and excreting dampness. After 1 w of treatment, blood flow velocity of femoral vein was observed by vascular ultrasound and venous thrombosis of lower extremities was checked by color Doppler ultrasound. Platelet (PLT), fibrinogen (FBG), VAS pain score and changes of limb circumference were detected before and after treatment in different groups. Results There was significant difference in blood flow velocity of femoral vein, PLT and Fbg between group A and group B 3 d and 7 d after operation (P<0.05). There was significant difference in VAS pain score between group A and group B 3 d after operation (P<0.05). Conclusion The therapy of tonifying qi and excreting dampness can effectively reduce the risk of lower extremity deep vein thrombosis after hip arthroplasty.
