目的：观察分析吻合器痔环切术（PPH）联合挂线治疗混合痔合并肛瘘的疗效。方法对2012年6月～2013年6月间收治的71例采用PPH联合挂线治疗的混合痔合并肛瘘患者的临床资料进行回顾性分析，观察PPH联合挂线治疗混合痔合并肛瘘的疗效。结果经术后随访，治愈率为100%（71/71），手术时间为（70.2±5.5）min，挂线脱落时间为（8.5±2.9）d，住院时间为11～17 d，平均（13.0±2.5）d，治疗后患者痔回缩满意，且肛瘘愈合完全。患者术后43例出现肛门功能不良，2例患者出现肛缘水肿，未有假性愈合、肛门狭窄、肛门失禁、吻合口狭窄等严重并发症发生。1例患者复发，复发率为1.41%（1/71）。结论PPH联合挂线治疗混合痔合并肛瘘的疗效显著，具有治愈率高、复发率低、并发症少的优点，值得临床推广应用。

목적：관찰분석문합기치배절술（PPH）연합괘선치료혼합치합병항루적료효。방법대2012년6월～2013년6월간수치적71례채용PPH연합괘선치료적혼합치합병항루환자적림상자료진행회고성분석，관찰PPH연합괘선치료혼합치합병항루적료효。결과경술후수방，치유솔위100%（71/71），수술시간위（70.2±5.5）min，괘선탈락시간위（8.5±2.9）d，주원시간위11～17 d，평균（13.0±2.5）d，치료후환자치회축만의，차항루유합완전。환자술후43례출현항문공능불량，2례환자출현항연수종，미유가성유합、항문협착、항문실금、문합구협착등엄중병발증발생。1례환자복발，복발솔위1.41%（1/71）。결론PPH연합괘선치료혼합치합병항루적료효현저，구유치유솔고、복발솔저、병발증소적우점，치득림상추엄응용。
Objective To observe and analyze the effect of PPH combined with seton treating mixed hemorrhoids with anal fistula. Methods 71 cases of using PPH combined with thread treatment in June 2012~June 2013 were retrospectively analyzed.The effect of PPH combined with seton therapy in the treatment of mixed hemorrhoids and anal fistula were analyzed. Results 71 cases of mixed hemorrhoid patients with anal fistula by using PPH combined with seton therapy were cured, and the cure rate was 100%(71/71), operation time(70.2 ± 5.5)minutes, wire hanging off time(8.5 ± 2.9) days, 11 days~17 days in thehospital, average(13 ± 2.5)days. 2 patients with postoperative perianal edema was occurred, 43 cases of anal dysfunction, no anal incontinence, anal stenosis, anastomotic stenosis and other serious complications. 2 patients had recurrence, the recurrence rate was 1.41(1/71). Conclusion The curative effect of PPH combined with seton therapy in the treatment of mixed hemorrhoids and anal fistula was significant,with high cure rate, low recurrence rate, less complications, which was worthy of clinical application.
