目的:观察5-氟尿嘧啶无痛微针阵列用于寻常疣治疗的临床疗效及对细胞免疫功能的影响。方法将60例寻常疣患者随机分为无痛微针阵列治疗组(微针治疗组)、涂药组及注射给药组,各20例,应用5-氟尿嘧啶等量用药治疗,每周2次,共12次;分别于治疗前、治疗后、治疗后3个月观察疗效及感染、疼痛、红肿、出血、瘙痒等不良反应和毒副作用。另于治疗前、后分别抽取微针治疗组患者及正常人对照组外周血,测定细胞免疫功能:CD4+、CD8+ T细胞数量(百分比)并进行比较。结果微针治疗组痊愈18例,显效2例,好转0例,无效0例,总有效率为100%;涂药组痊愈8例,显效6例,好转4例,无效2例,总有效率为70%;注射给药组痊愈12例,显效6例,好转2例,无效0例,总有效率为90%。微针治疗组疗效优于涂药组( P <0.05),而与注射给药组比较差异无统计学意义( P>0.05)。微针治疗组治疗前与正常人对照组的CD4+、CD8+ T细胞数量及CD4+/CD8+比值差异均有统计学意义;微针治疗后与治疗前比较差异亦有统计学意义。不良反应及复发率均以微针治疗组为最低。结论5-氟尿嘧啶无痛微针阵列用于寻常疣疗效显著,皮肤刺激性小,复发率低,并能改善细胞免疫功能。

목적:관찰5-불뇨밀정무통미침진렬용우심상우치료적림상료효급대세포면역공능적영향。방법장60례심상우환자수궤분위무통미침진렬치료조(미침치료조)、도약조급주사급약조,각20례,응용5-불뇨밀정등량용약치료,매주2차,공12차;분별우치료전、치료후、치료후3개월관찰료효급감염、동통、홍종、출혈、소양등불량반응화독부작용。령우치료전、후분별추취미침치료조환자급정상인대조조외주혈,측정세포면역공능:CD4+、CD8+ T세포수량(백분비)병진행비교。결과미침치료조전유18례,현효2례,호전0례,무효0례,총유효솔위100%;도약조전유8례,현효6례,호전4례,무효2례,총유효솔위70%;주사급약조전유12례,현효6례,호전2례,무효0례,총유효솔위90%。미침치료조료효우우도약조( P <0.05),이여주사급약조비교차이무통계학의의( P>0.05)。미침치료조치료전여정상인대조조적CD4+、CD8+ T세포수량급CD4+/CD8+비치차이균유통계학의의;미침치료후여치료전비교차이역유통계학의의。불량반응급복발솔균이미침치료조위최저。결론5-불뇨밀정무통미침진렬용우심상우료효현저,피부자격성소,복발솔저,병능개선세포면역공능。
Objective To observe the clinical efficacy and the influence on the cellular immune function of the treatment on verruca vulgaris by 5-fluorouracil painless microneedle array .Methods Sixty cases of verruca vulgaris patients were randomly divided into 3 groups :administration by painless microneedle ar-ray ,by applicator and by injection ,20 cases with each .The same amount of medication was provided , twice a week ,for a total of 12 times .Then the efficacy as well as the adverse reactions and the side effects ,such as infection ,pain ,red swelling ,bleeding or itching ,were observed before the treatment ,at the end of the treatment ,and 3 months after the end of the treatment ,respectively .Moreover ,cellular immune functions ,including the number (percentage) of CD4+ T-cells and CD8+ T-cells were determined by extracting the peripheral blood from the patients of the microneedle-array-administrated group before and after the treatment ,and were compared with 20 normal subjects .Results There were 18 well-healed , 2 excellent ,0 improved and 0 ineffective cases in the microneedle-array-administrated group ,with the ef-fective rate 100% ;8 cases were well-healed ,6 cases of excellent ,4 cases were improved and 2 cases were ineffective cases in the applicator group ,with the effective rate 70% ;12 cases were well-healed ,6 excel-lent ,2 cases were improved and 0 case was ineffective in the injection-administrated group ,with the effec-tive rate 90% .The microneedle-array-administrated group showed better efficacy than the applicator group (P <0 .05) ,while there was no significant difference between the efficacy of the the microneedle-array-ad-ministrated group and the injection-administrated group (P >0 .05) .Both the number of CD4+ T-cells , CD8+ T-cells and the ratio of CD4+ /CD8+ had significant difference between the the microneedle-array-ad-ministrated group before the treatment and the normal control group ,which also showed significant differ-ence after the treatment of microneedle array . In addition , the microneedle-array-administrated group showed the lowest incidence of both the adverse reactions or side effects and the recurrence rate .Conclu-sion T he treatment of verruca vulgaris by 5-fluorouracil painless microneedle array have significant clini-cal efficacy ,which also shows slighter skin irritation ,lower recurrence rate ,and can improve the cellular immune function .
