目的:探讨急性脑梗死患者外周血CD133+细胞的动态变化及其在评价脑梗死预后中的意义。方法采用流式细胞仪测定发病72 h以内的急性期脑梗死患者(梗死组80例)和有脑血管危险因素但无脑梗死发生的患者(对照组40例)外周血CD133+细胞水平,并记录脑梗死患者入院时、3 d、7 d和14 d外周血CD133+细胞水平和患者神经功能缺损评分(N I HSS评分)。结果急性梗死患者外周血 CD133+细胞百分比低于对照组,差异有统计学意义(P<0.05),且急性脑梗死患者外周血CD133+数量逐渐上升,第7天达高峰,脑梗死患者第7天CD133+细胞上升值与NI HSS评分改善值呈负相关(r=-0.673,P<0.05)。结论急性期脑梗死患者1周后外周血CD133+细胞明显升高提示预后良好,CD133+细胞对预测脑梗死预后具有一定意义。

Objective To explore the dynamic changes and significance of the level of peripheral blood CD133 positive (CD133+ ) cells in patients with acute cerbral infarction.Methods The level of peripheral blood CD133+ cells was determined by flow cytometrywithin 72 hours of onset of patients with acute cerebral infarction (infarct group ,n= 80)and patients with cerebrovascular risk factors without cerebral infarction (control group ,n=40). The neural function deficit score were deter-mined in patients with infarction group.Results The percentage of peripheral blood CD133+ cells in infarction patients was lower than that in the control group ,the difference was statistically significant (P<0.05). And the increment of the level of percentage of CD133+cells in peripheral blood of patients was negatively correlated with the improvement in NIHSS score at day 7 (r= -0.673 ,P<0.05).Conclusion The increase of the level of percentage of CD133+cells in peripheral blood after acute ischemic stroke is associated with good functional outcome. And the level of percentage of CD133+cells in peripheral blood has certain significance in predicting cerebral infarction outcome.
