[目的]探讨超声斑点追踪技术在评价肾衰竭患者心室旋转及扭转中的价值。[方法]选择本院肾内科诊治的肾功能衰竭患者30例(观察组)、健康志愿者30例(对照组),所有入选对象均接受心脏超声检查,应用斑点追踪成像技术测量各心肌层的感兴趣区并对其心肌组织进行追踪,比较两组左心室射血分数(LVEF)、左室舒张末期室间隔厚度(IVSTd)、舒张末期左室后壁厚度(LVPWd)及心尖水平及心底水平不同节段收缩期旋转角度峰值。[结果]观察组LVEF、IVSTd及LVPWd均值分别为(56.46±9.32)%、(1.24±0.24)cm及(1.12±0.11)cm ,与对照组的(66.33±4.32)%、(0.88±0.12)cm及(0.96±0.09)cm比较,差异均有显著性( P <0.05);观察组心尖水平的前间隔、前壁、侧壁、后壁、下壁及后间隔收缩期旋转角度峰值分别为(9.26±2.66)°、(9.03±3.18)°、(9.18±3.05)°、(9.72±2.82)°、(9.74±2.92)°、(9.58±3.65)°,均显著高于对照组,差异有统计学意义( P <0.05);观察组心底水平的后壁及下壁收缩期旋转角度峰值分别为(-7.76±3.12)°和(-9.15±3.16)°,均显著高于对照组的(-4.67±2.31)°和(-3.38±1.43)°,差异有统计学意义( P <0.05);经Pearson相关分析,肾衰竭患者心室扭转角度与LVEF间存在正相关(r=0.6782,P <0.05)。[结论]超声斑点追踪技术可准确的反映肾功能衰竭患者心室旋转及扭转,有较高的临床应用价值。

[목적]탐토초성반점추종기술재평개신쇠갈환자심실선전급뉴전중적개치。[방법]선택본원신내과진치적신공능쇠갈환자30례(관찰조)、건강지원자30례(대조조),소유입선대상균접수심장초성검사,응용반점추종성상기술측량각심기층적감흥취구병대기심기조직진행추종,비교량조좌심실사혈분수(LVEF)、좌실서장말기실간격후도(IVSTd)、서장말기좌실후벽후도(LVPWd)급심첨수평급심저수평불동절단수축기선전각도봉치。[결과]관찰조LVEF、IVSTd급LVPWd균치분별위(56.46±9.32)%、(1.24±0.24)cm급(1.12±0.11)cm ,여대조조적(66.33±4.32)%、(0.88±0.12)cm급(0.96±0.09)cm비교,차이균유현저성( P <0.05);관찰조심첨수평적전간격、전벽、측벽、후벽、하벽급후간격수축기선전각도봉치분별위(9.26±2.66)°、(9.03±3.18)°、(9.18±3.05)°、(9.72±2.82)°、(9.74±2.92)°、(9.58±3.65)°,균현저고우대조조,차이유통계학의의( P <0.05);관찰조심저수평적후벽급하벽수축기선전각도봉치분별위(-7.76±3.12)°화(-9.15±3.16)°,균현저고우대조조적(-4.67±2.31)°화(-3.38±1.43)°,차이유통계학의의( P <0.05);경Pearson상관분석,신쇠갈환자심실뉴전각도여LVEF간존재정상관(r=0.6782,P <0.05)。[결론]초성반점추종기술가준학적반영신공능쇠갈환자심실선전급뉴전,유교고적림상응용개치。
[Objective]To explore the value of ultrasound speckle tracking technology in the evaluation of renal failure patients with ventricular rotation and torsion .[Methods] Thirty patients with renal failure in our hospital(observation group) and 30 healthy volunteers(control group) were chosen .All subjects underwent echocardiography .Speckle tracking imaging technique was used to measure myocardial layer region of interest and track myocardial tissue .Left ventricular ejection fraction (LVEF) ,interventricular septum thickness at end diastole(IVSTd) ,left ventricular posterior wall depth(LVPWd) and different segments of systolic rotation angle at apex level and the cardiac base level were compared between two groups .[Results]Compared with the control group[(66 .33 ± 4 .32)% ,(0 .88 ± 0 .12)cm and (0 .96 ± 0 .09) ,respectively ] ,mean LVEF ,IVSTd and LVPWd of the observation group were (56 .46 ± 9 .32)% ,(1 .24 ± 0 .24)cm and (1 .12 ± 0 .11)cm re-spectively ,and there was significant difference ( P <0 .05) .Systolic interval rotation angle peak of anterior septal ,anterior wall ,lateral wall ,posterior wall ,inferior wall and posterior septal at apex level of the obser-vation group were (9 .26 ± 2 .66)° ,(9 .03 ± 3 .18)° ,(9 .18 ± 3 .05)° ,(9 .72 ± 2 .82)° ,(9 .74 ± 2 .92)° and (9 .58 ± 3 .65)° respectively ,which were all markedly higher than those of the control group ,and there were significant differences( P<0 .05) .Systolic interval rotation angle peak of posterior and inferior wall at cardiac base of the observation group were (-7 .76 ± 3 .12)° and (-9 .15 ± 3 .16)° respectively ,which were markedly higher than those of the control group ,and there were significant differences( P<0 .05) .Pearson correlation analysis showed a positive correlation between ventricular torsion angle and LVEF of patients with renal failure (r = 0 .6782 ,P<0 .05) .[Conclusion] Ultrasound speckle tracking technology can accurately reflect ventric-ular rotation and torsion of patients with renal failure in patients ,and has high application value .
