目的观察术毕前30min鼻内滴注右美托咪定对全身麻醉气管拔管期呛咳反应的影响。方法选取全身麻醉患者80例，年龄18~55岁，ASA分级I或II级，随机分为对照组（C组）和右美托咪定组（D组），每组30例，两组均静脉注射丙泊酚、芬太尼、顺式阿曲库铵麻醉诱导，术中用丙泊酚、瑞芬太尼维持。 D组术毕前30min鼻内滴注右美托咪定（0.5ug/kg）,C组鼻内滴注相同容量的生理盐水。记录苏醒时间，拔管时间及拔管即刻的Ramsay镇静评分和呛咳反应程度。结果C组拔管时Ramsay评分明显低于D组，呛咳反应程度高于D组,C组和D组苏醒时间、拔管时间差异没有统计学意义（P>0.05）。结论手术结束前30min鼻内滴注右美托咪定0.5ug/kg能降低拔管时的呛咳反应，提高麻醉苏醒质量。
목적관찰술필전30min비내적주우미탁미정대전신마취기관발관기창해반응적영향。방법선취전신마취환자80례，년령18~55세，ASA분급I혹II급，수궤분위대조조（C조）화우미탁미정조（D조），매조30례，량조균정맥주사병박분、분태니、순식아곡고안마취유도，술중용병박분、서분태니유지。 D조술필전30min비내적주우미탁미정（0.5ug/kg）,C조비내적주상동용량적생리염수。기록소성시간，발관시간급발관즉각적Ramsay진정평분화창해반응정도。결과C조발관시Ramsay평분명현저우D조，창해반응정도고우D조,C조화D조소성시간、발관시간차이몰유통계학의의（P>0.05）。결론수술결속전30min비내적주우미탁미정0.5ug/kg능강저발관시적창해반응，제고마취소성질량。
Objective To explore the ef ect of intranasal dexmedetomidine 30min postoperative on bucking during tracheal extubation under general anesthesia. Methods Sixty ASAI~II patients,18~55years old, under general anesthesia were randomly divided into two groups:normal saline control group(groupC,30cases) and dexmedetomidine group (group D,30cases).After anesthesia was induced with propofol, fantanil and cisatracarium, propofol and ruifentanil for maintenance. Group D intranasal 0.5μg/kg dexmedetomidine 30 min before the end of operation, and group C intranasal the equal volume of normal saline. plainesthesia time ,extubation time ,Ramsay score and extent of bucking reaction were recorded. Results Compared with patients in groupC; the stage of analepsia and extubation time were no significance; the Ramsay score were higher;the incidence of bucking was lower and its intensity were milder.Conclusion 30min operation before the end of the intranasal instil ation of dexmedetomidine 0.5μg/kg can reduce the extubation cough, improve the quality of anesthesia recovery.
