目的了解开放性手外伤术后伤口感染病原菌特点及感染相关因素,为感染干预提供依据。方法对2012年开放性手外伤住院患者进行回顾性调查与分析。结果802例开放性手外伤住院患者,术后伤口感染28例,感染发生率3.49%;共检出37株病原菌,革兰氏阴性菌检出率占75.68%,常见病原菌为铜绿假单胞菌、鲍氏不动杆菌占18.92%,其次为金黄色葡萄球菌为10.81%。感染率与患者年龄、伤后暴露时间、手术清创时间、术后留置引流管有显著相关性;结论本组手外伤术后伤口感染以革兰阴性菌为主;年龄大、伤后暴露时间长、手术时间长、伤口异物及创面引流是开放性手外伤术后感染危险因素。

목적료해개방성수외상술후상구감염병원균특점급감염상관인소,위감염간예제공의거。방법대2012년개방성수외상주원환자진행회고성조사여분석。결과802례개방성수외상주원환자,술후상구감염28례,감염발생솔3.49%;공검출37주병원균,혁란씨음성균검출솔점75.68%,상견병원균위동록가단포균、포씨불동간균점18.92%,기차위금황색포도구균위10.81%。감염솔여환자년령、상후폭로시간、수술청창시간、술후류치인류관유현저상관성;결론본조수외상술후상구감염이혁란음성균위주;년령대、상후폭로시간장、수술시간장、상구이물급창면인류시개방성수외상술후감염위험인소。
Objective To understand the open hand injuries postoperative wound infection pathogenic char-acteristics and infection-related factors, and provide the basis for the infection intervention .Retrospective survey and analysis methods open hand injuries in hospitalized patients in 2012 .Results 802 cases of open hand trauma hospitalized patients ,28 postoperative wound infection ,infection incidence of 3.49%;37 pathogens were detected , the detection rate of Gram-negative bacteria accounted for 75 .68% of com-mon pathogens as Pseudomonas aeruginosa bacteria boydii Acinetobacter accounted for 18 .92%, 10 . 81%, followed by Staphylococcus aureus .Infection rates and patient age ,injury , exposure time , the time of surgical debridement , postoperative indwelling drainage tube there is a significant correlation; Conclu-sion The group of hand injuries postoperative wound infection to gram-negative bacteria;older injury af-ter exposure a long time , a long time of surgery , wound foreign bodies open hand injuries postoperative in-fection risk factors and wound drainage .
