　　目的探讨改良简易外固定支架治疗高危股骨转子间骨折的疗效。方法自2006年6月-2011年6月收治高龄高危险股骨转子间骨折患者77例，年龄75-97岁，平均85.7岁，使用改良简易外固定支架固定治疗，并按照Harris评分进行评估。结果77例患者中75例完成随访，随访时间4-14个月，平均7.6个月，1例患者术后3周死亡，死于原有呼吸系统疾患。1例患者（转移癌病理性骨折）术后6周死亡，死于全身多器官功能衰竭。无固定钉折弯松动断裂脱落，无骨不愈合。根据Harris评分，优34例，良24例，可10例，差8例，优良率75.32%。结论改良简易外固定支架治疗高龄高危股骨转子间骨折是一种安全有效的方法。

Objective: To explore the clinical effect of the improved simple external fixator for the high-risk elderly patients with intertrochanteric fracture. Methods: From June 2006 to June 2011, 77 high-risk elderly patients (aged 75-97 years, mean 85.7 years old) with intertrochanteric fracture were treated by improved simple external fixator. and then evaluated by Harris score. Results: 75 cases of 77 patients completed fol ow-up, with 4-14 months of fol ow-up time , and average being 7.6 months. One patient died 3 weeks after surgery because of the original respiratory disorders . Another patient (pathologic fracture caused by metastatic carcinoma) died 6 weeks after surgery because of multiple organ failure. There were neither any complications of looseness or breakage of fixation, nor nonunion. According to Harris scoring system , the results were rated as excel ent in 34 cases, good in 24, fair in 10, and poor in 8, with the good-excel ent rate being 75.32%. Conclusion: Improved simple external fixator is a kind of safe and effective treatment for the high-risk elderly patients with intertrochanteric fracture.
