目的:探讨18 F-FDG PET/CT、CT在肝细胞肝癌TACE治疗后密实碘油病灶的临床价值。方法:经临床或病理确诊的26例肝细胞肝癌,TACE治疗后,分析18 F-FDG PET/CT对密实碘油病灶的判断结果,并与同期常规CT对比。结果:26例共112个碘油密实病灶,经病理、临床随访、动脉造影证实存活病灶11个,肿瘤坏死或抑制101个。常规CT诊断全部112个病灶为肿瘤抑制、肿瘤存活为0个病灶,准确率90.2%。18 F-FDG PET/CT诊断肿瘤坏死或抑制103个病灶、肿瘤存活为9个病灶,准确率97.3%。2组比较差异有统计学意义( P〈0.01)。结论:评价肝细胞肝癌TACE治疗后密实碘油区肿瘤状态,18 F-FDG PET/CT优于常规CT,能为进一步联合治疗提供依据,有重要的临床价值。

Objective: To evaluate the clinical value of 18F-FDG PET/CT and CT in the diagnosis of the complete lipiodol lesions in HCC after TACE . Methods:Twenty-six cases of clinically or pathologically confirmed HCC were treated with TACE. Then the results of complete lipiodol lesions of the 26 HCC cases after TACE were evaluated by 18F-FDG PET/CT and conventional CT respectively and were compared between these two groups. Results:Among the selected 112 complete lipiodol lesions of 26 HCC cases , there were 11 alive lesions and 101 necrotic lesions confirmed by pathology, angiography or clinical follow-up . Conventional CT indicated that all 112 lesions were necrosis with the diagnosis accuracy of 90. 2% . 18F-FDG PET/CT confirmed that 103 lesions were necrosis and 9 alive with the accuracy of 97. 3% . Statisti-cally significant difference was observed between these two groups (P<0. 01). Conclusion:18F-FDG PET/CT is superior to the conventional CT in evaluating the complete lipiodol lesions in HCC after TACE. Therefore, it is of great clinical value in the combination therapy for HCC.
