目的:研究HGB≥110 g/L无贫血症状的正常小儿从出生到学龄期(0岁~12岁)红细胞体积(MCV)的变化过程并探讨其对缺铁和缺铁性贫血(IDA)的预示作用。方法从实验室信息系统(LIS)随机导出我院2012年1月-2013年6月HGB≥110的917名0岁~12岁小儿血常规结果,分为8个年龄组,分析各年龄段MCV的变化,了解其与缺铁及IDA之间的关系。结果新生儿红细胞体积偏大(MCV 98.1~106.8 fL,正常82~92 fL),2,3个月渐渐恢复正常大小,4,5个月后开始变小,6个月~12个月最小(MCV 77.86 fL),之后慢慢变大,2岁后基本恢复正常。统计分析各年龄组小儿MCV大小明显不同,从各组MCV<82 fL的分布情况也可看出明显的差异。结论正常小儿MCV经历由大→正常→小→正常的变化过程,6个月~12个月时最小,预示着这个时期的小儿潜藏着缺铁的危险,如不及时强化铁的摄入,可能会引起IDA。

Objective To study the evolution of normal children's MCV from birth to school age (0-12 ) whose hemoglobins are all above 110g/L and to explore its indication role on iron deficiency and iron deficiency anemia (IDA). Methods To derive randomly the blood results of 917 children aged 0-12 ,HGB≥110 from LIS system from January of 2012 to June of 2013, divide into 8 age groups and analyze the change in MCV of all ages ,learning its relationship with iron deficiency and the IDA.Results Neonatus blood cell volume is larger than normal size (MCV 98.1-106.8 than 82-92), 2,3 month gradually returns to normal size, after 4,5 month it starts to become small, 6-12 month MCV is the smallest (MCV 77.86), then it slowly becomes big and three years later it returns normal. Statistical analysis on MCV the difference is significant between each age group, from the distribution of MCV <82 we can also see a 1significant difference. Conclusion normal children's MCV experiences by a large → Normal → small → normal process of change, 6-12 month is minimum, indicating that this period children have an danger of iron deficiency , if not fortified iron intake, maybe can cause IDA.
