目的 探讨Xper-CT与3D-DSA图像融合技术在合并血肿的小型脑动静脉畸形(AVM)显微手术中的作用.方法 回顾性分析2010年4月至2012年6月采用该技术指导显微手术的32例脑AVM破裂出血患者.术前DSA检查同时采集Xper-CT图像与三维图像,在后处理工作站进行图像融合.充分掌握术前信息后,全麻下行血肿清除加一期血管畸形切除术.结果 患者均顺利完成图像融合.根据融合图像,可显示血肿与供血动脉、畸形血管团及引流静脉的相对位置,判断出血的可能部位;设计手术骨窗的位置及范围,指导手术.术后DSA复查,畸形血管团均完全切除.结论 Xper-CT图像与3D-DSA图像融合技术在术前能从定位病变、规划手术入路等方面为破裂脑AVM显微手术提供重要信息,有助于手术顺利进行.
目的 探討Xper-CT與3D-DSA圖像融閤技術在閤併血腫的小型腦動靜脈畸形(AVM)顯微手術中的作用.方法 迴顧性分析2010年4月至2012年6月採用該技術指導顯微手術的32例腦AVM破裂齣血患者.術前DSA檢查同時採集Xper-CT圖像與三維圖像,在後處理工作站進行圖像融閤.充分掌握術前信息後,全痳下行血腫清除加一期血管畸形切除術.結果 患者均順利完成圖像融閤.根據融閤圖像,可顯示血腫與供血動脈、畸形血管糰及引流靜脈的相對位置,判斷齣血的可能部位;設計手術骨窗的位置及範圍,指導手術.術後DSA複查,畸形血管糰均完全切除.結論 Xper-CT圖像與3D-DSA圖像融閤技術在術前能從定位病變、規劃手術入路等方麵為破裂腦AVM顯微手術提供重要信息,有助于手術順利進行.

Objective To evaluate the applicative value of Xper-CT and 3-dimension DSA image fusion technology in guiding the microsurgery of Small ruptured cerebral arteriovenous malformation.Methods The imaging and clinical data of 32 cases,which were diagnosed as ruptured cerebral arteriovenous malformation from April 2010 to June 2012,were retrospectively analyzed.Xper-CT images and the 3D-DSA images were obtained by preoperative DSA examination at the same time.The fusion images were achieved in the post-processing workstation,which providing many useful information in guiding the microsurgery.Results All patients were successfully completed 3D-DSA and Xper-CT image fusion.The fusion image can show the relationship and orientation between the hematoma and the supplying arteries,nidus and draining veins.We can get much useful information related to the operation according to the fusion image,such as determining the range of bone flap,judging the location of AVM,and bleeding point.All the patients showed total resection of AVM by postoperative DSA.Conclusions Preoperative Xper-CT and 3D-DSA image fusion technology can provide much important information such as lesion location and planning the approach of surgery.It is helpful to complete the microsurgery smoothly and increase the safety of the procedure.
