目的:测定类风湿关节炎(RA)周围神经病变患者血清中血管内皮生长因子(VEGF)水平,并探讨其临床意义。方法收集32例RA合并周围神经病变患者(RA1组)及年龄、性别、病程相匹配的30例RA不合并周围神经病变患者(RA2组),以上两组合并为RA组,同时选择性别、年龄匹配的健康体检者30例为正常对照组,用ELISA方法检测全部研究对象的血清VEGF水平,并分析其与临床及实验室指标的相关性。结果(1)RA患者的VEGF水平明显高于正常对照组[(148.25±91.87) ng/ml vs.(44.50±17.06) ng/ml , P<0.001];RA1组明显高于 RA2组[(172.61±96.84)ng/ml vs.(118.28±76.71)ng/ml,P<0.05];(2)RA1组VEGF与红细胞沉降率、C反应蛋白(CRP)、血小板呈正相关(r=0.365,P=0.003;r=0.431,P=0.013;r=0.425,P=0.001),与白蛋白呈负相关(r=-0.329,P=0.011)。RA组VEGF与红细胞沉降率、CRP、血小板呈正相关(r=0.312,P=0.007;r=0.351,P=0.007;r=0.580,P=0.001),与白蛋白呈负相关(r=-0.428, P=0.001),与其他指标无相关性(P>0.05)。(3)神经重度损伤组与轻中度损伤组 VEGF 无统计学差异[(179.59±138.03)ng/ml vs.(140.50±78.25)ng/ml,P>0.05];(4)上肢受累[(165.19±77.09) ng/ml]、下肢受累[(173.09±79.81)ng/ml]及上下肢均受累[(135.74±63.22)ng/ml]的RA周围神经病变患者VEGF水平两两比较均无统计学差异(均P>0.05)。结论 RA合并周围神经病变患者血清VEGF明显高于不合并神经病变的RA患者,提示VEGF可能与提示RA患者存在周围神经病变有关,但不能提示神经受损程度和损伤区域。

목적:측정류풍습관절염(RA)주위신경병변환자혈청중혈관내피생장인자(VEGF)수평,병탐토기림상의의。방법수집32례RA합병주위신경병변환자(RA1조)급년령、성별、병정상필배적30례RA불합병주위신경병변환자(RA2조),이상량조합병위RA조,동시선택성별、년령필배적건강체검자30례위정상대조조,용ELISA방법검측전부연구대상적혈청VEGF수평,병분석기여림상급실험실지표적상관성。결과(1)RA환자적VEGF수평명현고우정상대조조[(148.25±91.87) ng/ml vs.(44.50±17.06) ng/ml , P<0.001];RA1조명현고우 RA2조[(172.61±96.84)ng/ml vs.(118.28±76.71)ng/ml,P<0.05];(2)RA1조VEGF여홍세포침강솔、C반응단백(CRP)、혈소판정정상관(r=0.365,P=0.003;r=0.431,P=0.013;r=0.425,P=0.001),여백단백정부상관(r=-0.329,P=0.011)。RA조VEGF여홍세포침강솔、CRP、혈소판정정상관(r=0.312,P=0.007;r=0.351,P=0.007;r=0.580,P=0.001),여백단백정부상관(r=-0.428, P=0.001),여기타지표무상관성(P>0.05)。(3)신경중도손상조여경중도손상조 VEGF 무통계학차이[(179.59±138.03)ng/ml vs.(140.50±78.25)ng/ml,P>0.05];(4)상지수루[(165.19±77.09) ng/ml]、하지수루[(173.09±79.81)ng/ml]급상하지균수루[(135.74±63.22)ng/ml]적RA주위신경병변환자VEGF수평량량비교균무통계학차이(균P>0.05)。결론 RA합병주위신경병변환자혈청VEGF명현고우불합병신경병변적RA환자,제시VEGF가능여제시RA환자존재주위신경병변유관,단불능제시신경수손정도화손상구역。
Objective To study the expression of VEGF in the peripheral blood of rheumatoid patients with peripheral neuropathy. Methods Thirty two rheumatoid patients with peripheral neuropathy (experimental group) and 30 rheumatoid patients without peripheral neuropathy (RA group) and 30 healthy controls were enrolled in the study. The level of VEGF in the peripheral blood were detected using ELISA method to analyze the relation of clinical and laboratory indicators. Results (1)The level of the VEGF in experimental group [(148.25±91.87)ng/ml] was higher than RA group [(44.50±17.06)ng/ml, P<0.001]. The level of VEGF of RA1 group was higher than RA2 group[(172.61±96.84)ng/ml vs. (118.28± 76.71)ng/ml, P<0.001]. (2)The level of VEGF in RA1 group was positive correlation with ESR, CRP, PLT (r=0.365, P=0.003; r=0.431, P=0.013; r=0.425, P=0.001). While negative with ALB (r=-0.329, P=0.011). The level of VEGF in RA group was positive correlation with ESR, CRP, PLT (r=0.312, P=0.007; r=0.351, P=0.007; r=0.580, P=0.001) and negative with ALB (r=-0.428, P=0.001). (3)The level of the VEGF between severe group [(179.59±138.03)ng/ml] and soft group [(140.50±78.25)ng/ml] had no significant difference. (4)The level of VEGF of patients who peripheral neuropathy affected only upper limbs or lower limbs or both upper and lower limbs has no no significant difference [(165.19±77.09)ng/ml, (173.09±79.81)ng/ml, (135.74±63.22)ng/ml, all P>0.05]. Conclusion VEGF may be an important diagnosis indicators to rheumatoid arthritis with peripheral neuropathy, but can not indicate the level and position of peripheral neuropathy.
