目的：探讨采用经掌侧入路斜T形钢板内固定治疗桡骨远端不稳定骨折的临床疗效。方法将82例桡骨远端不稳定骨折患者按数字随机法分为两组，每组各41例。治疗组采用经掌侧入路斜T形钢板内固定治疗，对照组采用经掌侧入路解剖钢板内固定治疗。比较两组手术时间、术中出血量、住院时间及骨折愈合时间，手术前后掌倾角、尺偏角及桡骨高度，术后对两组患者随访12个月，比较腕关节功能优良率。结果两组手术时间、术中出血量及住院时间比较，差异无统计学意义（ P >0.05）；治疗组骨折愈合时间优于对照组，差异有统计学意义（ P <0.05）；掌倾角、尺偏角及桡骨高度两组术前差异无统计学意义，治疗后两组均显著改善，治疗后两组比较，治疗组优于对照组，差异有统计学意义（ P <0.05）；两组腕关节优良率比较，治疗组优于对照组，差异有统计学意义（ P <0.05）。结论经掌侧入路斜T形钢板内固定治疗桡骨远端不稳定骨折疗效确切，可将骨折端进行良好复位和坚强固定，有利于腕关节功能的恢复，值得临床推广应用。
목적：탐토채용경장측입로사T형강판내고정치료뇨골원단불은정골절적림상료효。방법장82례뇨골원단불은정골절환자안수자수궤법분위량조，매조각41례。치료조채용경장측입로사T형강판내고정치료，대조조채용경장측입로해부강판내고정치료。비교량조수술시간、술중출혈량、주원시간급골절유합시간，수술전후장경각、척편각급뇨골고도，술후대량조환자수방12개월，비교완관절공능우량솔。결과량조수술시간、술중출혈량급주원시간비교，차이무통계학의의（ P >0.05）；치료조골절유합시간우우대조조，차이유통계학의의（ P <0.05）；장경각、척편각급뇨골고도량조술전차이무통계학의의，치료후량조균현저개선，치료후량조비교，치료조우우대조조，차이유통계학의의（ P <0.05）；량조완관절우량솔비교，치료조우우대조조，차이유통계학의의（ P <0.05）。결론경장측입로사T형강판내고정치료뇨골원단불은정골절료효학절，가장골절단진행량호복위화견강고정，유리우완관절공능적회복，치득림상추엄응용。
Objective To investigate the curative effect of volar T-shaped plate internal fixation for unstable distal radius fractures. Methods Eighty-two patients with unstable distal radius fractures were divided into two groups by random number table method,41 cases were in each group. The treatment group was treated with volar oblique T-shaped plate internal fixation,and the control group with volar anatomical plate internal fixation. The operation time,amount of bleeding,hospitalization time and healing time of fracture were compared between the two groups;the palmar tilt,ulnar deviation and radial height before and after operation were compared. Patients of the two groups were followed up for 12 months after operation,the excellent and good rate of wrist joint function was compared. Results There were no significant differences in the operation time,amount of bleeding and hospitalization time of patients between two groups(all P >0. 05). The fracture healing time in the treat-ment group was shorter than that of the control group,the difference was statistically significant( P <0. 05). Before operation,there were no sig-nificant differences in the palmar tilt,ulnar deviation and radial height between two groups;after operation,the palmar tilt,ulnar deviation and ra-dial height were improved in both groups,but better improvement was observed in treatment group(all P <0. 05). The excellent rate of wrist joint function was higher in the treatment group( P <0. 05). Conclusion The volar T-shaped plate internal fixation for unstable distal radius frac-tures has an exact curative effect,the fracture end can have a good reduction and rigid fixation,it is conducive to the recovery of wrist function.
