目的:探讨大容量双肺灌洗治疗肺泡蛋白沉积症的临床效果。方法对10例肺泡蛋白沉积症患者,在静脉复合麻醉下行双腔支气管插管,双肺成功隔离后,一侧肺纯氧通气,另一侧肺采用灌洗液反复灌洗,于灌洗前及灌洗后分别测定血气、肺功能,并观察术后病人临床症状改善情况。结果成功进行了大容量双肺灌洗,术中顺利,术后临床症状明显减轻,无明显术后并发症。灌洗治疗后动脉血氧分压、肺通气功能和肺肺弥散功与治疗前对比改变有统计学意义;相反,动脉血二氧化碳分压在灌洗治疗前后无统计学意义。结论大容量双肺灌洗治疗肺泡蛋白沉积症病是一种安全、有效的科学方法。

목적:탐토대용량쌍폐관세치료폐포단백침적증적림상효과。방법대10례폐포단백침적증환자,재정맥복합마취하행쌍강지기관삽관,쌍폐성공격리후,일측폐순양통기,령일측폐채용관세액반복관세,우관세전급관세후분별측정혈기、폐공능,병관찰술후병인림상증상개선정황。결과성공진행료대용량쌍폐관세,술중순리,술후림상증상명현감경,무명현술후병발증。관세치료후동맥혈양분압、폐통기공능화폐폐미산공여치료전대비개변유통계학의의;상반,동맥혈이양화탄분압재관세치료전후무통계학의의。결론대용량쌍폐관세치료폐포단백침적증병시일충안전、유효적과학방법。
Objective To explore the clinical outcomes of Massive whole lung lavage for the therapy of pulmonary alveolar proteinosis. Methods 10 patients with pulmonary alveolar proteinosis at various stage were performed double lumen endotracheal intubation under the condition of combined intravenous anesthesia, and the bilateral lungs were successfully isolated from each other. Ventilation with oxygen was carried out to one lateral lung and repeated lavage was meanwhile conducted to the other lateral lung. In the end, both the bilateral lungs were given lavage. Blood gas and lung function were detected before and after lavage, Clinical observation on postoperative patients symptom improvement.Results Successful bilateral lung lavage was conducted in the 10 patients without obvious postoperative complications. The change in arterial partial pressure of oxygen (PaO2) 、lung ventilatory fuction and lung diffuse fuction was statistically sign ificant when it was compared between before and after whole lung lavage . on the contrary, there were no obvious differences in arterial partial pressure of carbon dioxide (PaCO2) between before and after lavage . Conclusion Massive Whole lung lavage is effective on pulmonary alveolar proteinosis with safety.
