目的:探讨在内镜辅助下经鼻蝶入路手术治疗垂体瘤的临床效果。方法随机选取2008年2月至2011年12月采用在内镜辅助下经鼻蝶入路手术治疗垂体瘤89例病例资料,术前均行影像学及血液内分泌激素等相关检查,且均在内镜下经鼻蝶手术切除病变,并经随访12至18个月后分析临床治疗效果。结果48例(53.93%),次全切33例(37.08%),部分切除8例(8.99%),所有患者症状均有不同程度改善,术后无严重并发症,随访无复发病例。结论内镜辅助下经鼻蝶入路手术治疗垂体腺方式借助内镜及显微镜可以使视野更清晰,照明更充分,最大限度地避免术中的误伤,更准确地切除肿瘤,且并发症更少,此种手术治疗方法值得临床推广。

목적:탐토재내경보조하경비접입로수술치료수체류적림상효과。방법수궤선취2008년2월지2011년12월채용재내경보조하경비접입로수술치료수체류89례병례자료,술전균행영상학급혈액내분비격소등상관검사,차균재내경하경비접수술절제병변,병경수방12지18개월후분석림상치료효과。결과48례(53.93%),차전절33례(37.08%),부분절제8례(8.99%),소유환자증상균유불동정도개선,술후무엄중병발증,수방무복발병례。결론내경보조하경비접입로수술치료수체선방식차조내경급현미경가이사시야경청석,조명경충분,최대한도지피면술중적오상,경준학지절제종류,차병발증경소,차충수술치료방법치득림상추엄。
Objective To explore the clinical effect of endoscopic assisted approach in the treatment of pituitary adenomas transsphenoidal operation. Methods randomly selected in 2008 February to 2011 December in the endoscopic endonasal transsphenoidal operation for pituitary adenoma 89 cases, preoperative imaging and blood endocrine hormone related examination, and were in the endoscopic endonasal transsphenoidal operation excision of the lesion, and were followed up for 12 to 18 months after the clinical analysis effect of treatment. Results 48 cases (53.93%), and cut all 33 patients (37.08%), partial nephrectomy 8 cases (8.99%), all patients symptoms were improved to varying degrees, no serious complications, postoperative follow-up cases without recurrence. Conclusion endoscope assisted by nasal adjustment in the way of surgical treatment of pituitary gland with endoscope and microscope can make clearer view, lighting, more fully maximize prevent injured in operation and more accurate.
