Primary acute angle-closure glaucoma%Cyclodialysis%Ultrasound biomicroscopy
目的:探讨超声生物显微镜(UBM)在原发性急性闭角型青光眼并发睫状体脱离诊断中的临床意义。方法:选取2008年8月-2012年12月本院眼科诊断为急性闭角型青光眼(急性发作期)患者138例共169只眼作为研究对象,积极降低患者眼压,并对UBM检查发现的睫状体脱离患者进行分析和总结。结果:169只眼(急性发作期)中有43只眼并发睫状体脱离(25.44%);发生睫状体脱离者与未发生者在年龄、性别上差异无统计学意义(P>0.05),且两者比较,角膜厚度、前房深度、巩膜与睫状体夹角差异均无统计学意义(P>0.05),睫状体厚度、小梁与睫状体距离差异有统计学意义(P<0.05);睫状体脱离患者前房角镜检查示房角均未完全关闭,至少有2个时钟点未关闭。结论:UBM可以对睫状体脱离作出客观准确的判断,指导急性闭角型青光眼术前的治疗并随访观察,具有较高的诊断价值。
目的:探討超聲生物顯微鏡(UBM)在原髮性急性閉角型青光眼併髮睫狀體脫離診斷中的臨床意義。方法:選取2008年8月-2012年12月本院眼科診斷為急性閉角型青光眼(急性髮作期)患者138例共169隻眼作為研究對象,積極降低患者眼壓,併對UBM檢查髮現的睫狀體脫離患者進行分析和總結。結果:169隻眼(急性髮作期)中有43隻眼併髮睫狀體脫離(25.44%);髮生睫狀體脫離者與未髮生者在年齡、性彆上差異無統計學意義(P>0.05),且兩者比較,角膜厚度、前房深度、鞏膜與睫狀體夾角差異均無統計學意義(P>0.05),睫狀體厚度、小樑與睫狀體距離差異有統計學意義(P<0.05);睫狀體脫離患者前房角鏡檢查示房角均未完全關閉,至少有2箇時鐘點未關閉。結論:UBM可以對睫狀體脫離作齣客觀準確的判斷,指導急性閉角型青光眼術前的治療併隨訪觀察,具有較高的診斷價值。
목적:탐토초성생물현미경(UBM)재원발성급성폐각형청광안병발첩상체탈리진단중적림상의의。방법:선취2008년8월-2012년12월본원안과진단위급성폐각형청광안(급성발작기)환자138례공169지안작위연구대상,적겁강저환자안압,병대UBM검사발현적첩상체탈리환자진행분석화총결。결과:169지안(급성발작기)중유43지안병발첩상체탈리(25.44%);발생첩상체탈리자여미발생자재년령、성별상차이무통계학의의(P>0.05),차량자비교,각막후도、전방심도、공막여첩상체협각차이균무통계학의의(P>0.05),첩상체후도、소량여첩상체거리차이유통계학의의(P<0.05);첩상체탈리환자전방각경검사시방각균미완전관폐,지소유2개시종점미관폐。결론:UBM가이대첩상체탈리작출객관준학적판단,지도급성폐각형청광안술전적치료병수방관찰,구유교고적진단개치。
Objective:To investigate clinical the significance of diagnosis about Primary acute angle-closure glaucoma concurrent under ultrasound biomicroscopy.Method:Selected 138 cases of 169 eyes with acute angle-closure glaucoma(acute phase)as the research objects ,from August 2008 to December 2012 in our hospital. Actively reduced intraocular pressure,analyzed and summarized the patients with cyclodialysis by ultrasound biomicroscopy. Result:It was improved in 43 eyes(25.44%)of all the 169 eyes,there were no statistical differences in age or sex between the patient with cyclodialysis and those without(P>0.05);there was no statistical significance in corneal thickness,the anterior chamber depth and scleral with ciliary body angle between the patient with cyclodialysis and those without(P>0.05);the difference of ciliary body thickness and the distance of the trabecular and ciliary body between he patient with cyclodialysis and those without had statistical significance(P<0.05);gonioscopy showed Angle-closure in the patient with cyclodialysis incompletely,there were 2-o’clock at least. Conclusion:The ultrasound biomicroscopy do good to diagnosing the cyclodialysis accurately,guiding treatment of primary acute angle-closure glaucoma,which has higher diagnostic value.
