目的探讨显微外科治疗双侧精索静脉曲张不育患者的临床应用价值。方法我科于2008年6月—2011年6月对45例确诊为双侧精索静脉曲张不育患者行显微镜下双侧精索静脉结扎术。结果显微镜下双侧精索静脉结扎术后2年有32例患者配偶（71.11%）获得妊娠，妊娠率以及术后阴囊水肿、复发和睾丸萎缩等并发症发生率优于其他手术方式。妊娠主要集中在术后12个月内，术后15个月妊娠概率几乎为零。结论男性不育患者双侧精索静脉曲张的发病率高，与其他术式比较，显微外科治疗双侧精索静脉曲张不育患者疗效更好，并发症更少，具有很高的临床应用价值。术后超过15个月患者配偶仍未妊娠者建议采取其他诊疗方法。
목적탐토현미외과치료쌍측정색정맥곡장불육환자적림상응용개치。방법아과우2008년6월—2011년6월대45례학진위쌍측정색정맥곡장불육환자행현미경하쌍측정색정맥결찰술。결과현미경하쌍측정색정맥결찰술후2년유32례환자배우（71.11%）획득임신，임신솔이급술후음낭수종、복발화고환위축등병발증발생솔우우기타수술방식。임신주요집중재술후12개월내，술후15개월임신개솔궤호위령。결론남성불육환자쌍측정색정맥곡장적발병솔고，여기타술식비교，현미외과치료쌍측정색정맥곡장불육환자료효경호，병발증경소，구유흔고적림상응용개치。술후초과15개월환자배우잉미임신자건의채취기타진료방법。
Objective To investigate the clinical effects of the macroscopic subinguinal varicocelectomy treating 45 cases of the infertility male patients with bilateral varicocele. Methods 45 cases infertility male patients,undergoing bilateral varicocele,were treated with the macroscopic subinguinal varicocelectomy,then were followed up 2 years. Results 32 patients'spouse (71.11%)were pregnanct after the macroscopic subinguinal varicocelectomy.The rate of pregnancy and complications such as scrotal edema,reaccurent varicocele and atrophia testiculi,was much better than that of other operations.However,31 patients'spouse were pregnanct in 12 months after after the macroscopic subinguinal varicocelectomy,and the last one was pregnanct in 15 months.Conclusion The infertility males patients have a strikingly high incidence of bilateral varicocele.Compared with other operations,the macroscopic subinguinal varicocelectomy have better effect on them and can significantly reduce the incidence of complications,but they were suggested to treated by other therapy if it was ineffective in 15 months after operation.
