Donepezil hydrochloride%Cerebral vascular spasm after subarachnoid hemorrhage%Cognitive impairment
目的观察盐酸多奈哌齐片治疗蛛网膜下隙出血后血管痉挛致认知功能障碍的疗效。方法选取蛛网膜下隙出血后血管痉挛致认知功能障碍患者60例,随机分为观察组和对照组30例。两组患者均经神经内科常规药物治疗,包括降颅内压、止血、解痉、控制血压、血糖、营养脑细胞等。观察组给予盐酸多奈哌齐片75 mg 1次/d。治疗前后评定患者简易智能精神状态量表(MMSE)、临床痴呆评定量表(CDR)和日常生活能力量表(ADL),并对两组患者进行比较。结果治疗后,观察组患者MMSE评分与对照组比较明显提高,CDR、ADL评分与对照组比较有明显降低,差异有统计学意义(P<0.05);2组患者均无出现明显不良反应,对比无显著差异性(P>0.05)。结论盐酸多奈哌齐片治疗蛛网膜下隙出血后血管痉挛致认知功能障碍疗效确切,无明显副作用。

목적관찰염산다내고제편치료주망막하극출혈후혈관경련치인지공능장애적료효。방법선취주망막하극출혈후혈관경련치인지공능장애환자60례,수궤분위관찰조화대조조30례。량조환자균경신경내과상규약물치료,포괄강로내압、지혈、해경、공제혈압、혈당、영양뇌세포등。관찰조급여염산다내고제편75 mg 1차/d。치료전후평정환자간역지능정신상태량표(MMSE)、림상치태평정량표(CDR)화일상생활능역량표(ADL),병대량조환자진행비교。결과치료후,관찰조환자MMSE평분여대조조비교명현제고,CDR、ADL평분여대조조비교유명현강저,차이유통계학의의(P<0.05);2조환자균무출현명현불량반응,대비무현저차이성(P>0.05)。결론염산다내고제편치료주망막하극출혈후혈관경련치인지공능장애료효학절,무명현부작용。
Objective To evaluate the therapeutic effect of cognitive impairment improvement in symptomatic cerebral vasospam following subarachnoid hemorrhage patients under treatment of donepezil hydrochloride tablets. Methods 60 patients with symptomatic cerebral vasospam induced cognitive impairment following subarachnoid hemorrhage were included in this trial and all patients were randomly divided into two groups, with the observation group 30 cases and the control group 30 cases. Patients in observation group were treated with donepezil hydrochloride tablets 75 mg qd after inclusion. Both of the observation group and the control group were given the same nervous internal routine treatment, including reducing intracranial pressure, relieving spasm, controlling blood pressure and blood sugar levels, and nourishing brain cells. We assessed the patient state before arid after treatment using the following scales, Mini-mental state examination ( MMSE ), Clinical Dementia Rating ( CDR ) and Activity of Daily Living Scale ( ADL ). Results Patients treated with donepezil hydrochloride tablets appeared obvious elevation in MMSE scoring(P<0.01), and significant reduction was also detected in CDR and ADL scoring(P<0.05).Two group of patients without obvious adverse reaction occurred, compared to no significant difference (P>0.05). Conclusion Treatment of donepezil hydrochloride tablets for cognitive impairment improvement patients with Subarachnoid hemorrhage is an effective method and is worthy of recommending.
