目的探讨闭合复位防旋股骨近端髓内钉(PFNA)内固定治疗高龄股骨粗隆间骨折的疗效和安全性。方法分析我院2011年1月-2012年6月间采用闭合复位PFNA内固定(PFNA组，n=30)和股骨近端锁定加压钢板(LCP组，n=30)治疗的60例高龄股骨粗隆间骨折患者的临床资料。结果 PFNA组患者的显性出血量、住院时间、负重时间、骨折愈合时间和并发症率都少于LCP组(P <0.05)，疗效优于LCP组(P <0.05)，两组患者的手术时间无显著差异(P >0.05)。结论闭合复位PFNA内固定治疗股骨粗隆间骨折具有疗效确切、安全可靠、并发症少等优点，可以作为治疗股骨粗隆间骨折的优先方案。

Objective To investigate the curative effect and safety of the closed reduction and internal fixation using proximal femoral nail anti-rotation(PFNA) for the treatment in elderly patients with femoral intertrochanteric fractures. Methods Analyzed retrospectively the clinical data of the closed reduction and internal fixation using PFNA(PFNA group, n=30) or locking compression plate(LCP group, n=30) for treatment of 60 elderly patients with femoral intertrochanteric fractures in our hospital from January 2011 to June 2012. Results The amount of bleeding, hospital stays, load time, fracture healing time, and complication rate in the PFNA group were significant lower than those in the LCP group(P <0.05). Moreover, curative effect in the PFNA group was significant better than that in the LCP group(P <0.05). However, there was no significant difference in operation time between two groups(P >0.05). Conclusions Closed reduction and internal fixation using PFNA for the treatment in elderly patients with femoral intertrochanteric fractures has a lot of advantages, including effective, safety, less complication. It can be as preferred method to treat intertrochanteric fracture of femur.
