目的探讨研究胫骨 pilon 骨折的手术内固定方式的选择。方法我院在2008年8月至2012年2月期间共接诊80例胫骨 pilon 骨折患者,将其随机分成实验组及对照组,其中对照组40例给予有限切开内固定并结合石膏外固定进行固定,实验组40例则给予切开复位进行内固定进行治疗,对比分析两组的治疗效果、骨折愈合时间及不良反应。结果随访半年至2年,对照组治疗总有效29例,总有效率为72.5%,实验组治疗总有效39例,总有效率为97.5%,实验组明显高于对照组,差异显著具有统计学意义(P <0.05),实验组患者骨折的愈合时间为(16±2.4)周明显小于对照组(20±2.3)周,差异显著具有统计学意义(P <0.05),实验组患者的并发症发生率明显低于对照组,差异显著具有统计学意义(P <0.05)。结论对于胫骨 pilon 骨折患者来说,切开复位进行内固定疗法可有效提高其治疗效果及降低并发症的发生率,应在临床中进行推广。

목적탐토연구경골 pilon 골절적수술내고정방식적선택。방법아원재2008년8월지2012년2월기간공접진80례경골 pilon 골절환자,장기수궤분성실험조급대조조,기중대조조40례급여유한절개내고정병결합석고외고정진행고정,실험조40례칙급여절개복위진행내고정진행치료,대비분석량조적치료효과、골절유합시간급불량반응。결과수방반년지2년,대조조치료총유효29례,총유효솔위72.5%,실험조치료총유효39례,총유효솔위97.5%,실험조명현고우대조조,차이현저구유통계학의의(P <0.05),실험조환자골절적유합시간위(16±2.4)주명현소우대조조(20±2.3)주,차이현저구유통계학의의(P <0.05),실험조환자적병발증발생솔명현저우대조조,차이현저구유통계학의의(P <0.05)。결론대우경골 pilon 골절환자래설,절개복위진행내고정요법가유효제고기치료효과급강저병발증적발생솔,응재림상중진행추엄。
Objective To research the mode selection of internal fixation of pilon fracture. Method There were 80 cases of tibial Pilon fracture, they were divided into experimental group and control group, the control group were given limited open reduction and combined with plaster external fixed, experimental group were given open reduction and internal fixation, analysis treatment effect, fracture healing time and adverse reaction of the two groups. Results Follow-up of half a year to 2 years, in the control group, the total effective treatment is 29 cases, the total effective rate was 72.5%; in experimental group, the total effective treatment is 39 cases, the total effective rate was 97.5%, significantly higher than the control group, with significant difference (P<0.05); the patients in experimental group with fracture healing time for (16±2.4) weeks obviously less than control group (20±2.3) weeks, with significant difference (P<0.05); the incidence of complications of patients was significantly lower than the control group, with significant difference (P<0.05). Conclution For tibial pilon fractures, open reduction and Internal fixation can effectively improve the treatment effect and reduce the incidence of complications, it should be in the clinical promotion.
