Nt-ProBNP%Differentiation of dyspnea%Heart failure
目的探讨N-末端脑钠肽(Nt-ProBNP)在急性呼吸困难的鉴别诊断价值。方法测定192例急性呼吸困难患者血浆Nt-ProBNP浓度,采用超声心动图检查左室射血分数(LVEF),依据相关检查,分为心源性呼吸困难(cardiac dyspnea,CD)组与非心源性呼吸困难(non-cardiac dyspnea,NCD)组,观察两种急性呼吸困难患者的Nt-ProBNP浓度,同时依据不同LVEF等级进行Nt-ProBNP比较。结果 CD组患者Nt-ProBNP浓度明显高于NCD组患者(P<0.01),Nt-ProBNP水平与NYHA分级具有正相关性,与LVEF呈负相关性(P<0.01)。结论 Nt-ProBNP水平检测对于急性呼吸困难的鉴别及患者心功能的评估有重要意义。

목적탐토N-말단뇌납태(Nt-ProBNP)재급성호흡곤난적감별진단개치。방법측정192례급성호흡곤난환자혈장Nt-ProBNP농도,채용초성심동도검사좌실사혈분수(LVEF),의거상관검사,분위심원성호흡곤난(cardiac dyspnea,CD)조여비심원성호흡곤난(non-cardiac dyspnea,NCD)조,관찰량충급성호흡곤난환자적Nt-ProBNP농도,동시의거불동LVEF등급진행Nt-ProBNP비교。결과 CD조환자Nt-ProBNP농도명현고우NCD조환자(P<0.01),Nt-ProBNP수평여NYHA분급구유정상관성,여LVEF정부상관성(P<0.01)。결론 Nt-ProBNP수평검측대우급성호흡곤난적감별급환자심공능적평고유중요의의。
Objective?To investigate the diagosis value of Nt-proBNP in differentiation of dyspnea. Methods Plasma Nt-ProBNP level of 192 cases of emergency dyspnea patients was measured. Echacardiography was used to detect their left ventricular ejection fraction (LVEF). According to the related examination, patients were divided into cardiac dyspnea(CD)group and non-cardiac dyspnea(NCD)group, and Nt-ProBNP level was compared between two groups. Menwhile, the Nt-ProBNP concentraton in different LVEF level was compared. Results The Nt-ProBNP level in CD group was obviously higher than that in NCD group(P<0.01). There was a positive correlation between Nt-proBNP level and NYHA grading, and there was a negative correlation between Nt-proBNP level and LVEF(P<0.01).Conclusion Nt-ProBNP can be used for the identification of emergency dyspnea, and make the assessment of cardiac function of patients.
