目的探讨64排螺旋CT( MSCT)的三维重建术在颅骨骨折的应用价值。方法对2013年1~8月本院颅脑外伤患者252例行MSCT扫描,扫描资料送到Philips工作站EBW进行容积再现( VR)、表面密度投影( SurfaceMIP)处理,由2名有经验的放射科医生盲法评估轴位骨窗、VR及SurfaceMIP三维重建图像对骨折的检出情况。结果轴位骨窗、VR及SurfaceMIP检出骨折例数分别为25例、41例及49例,轴位骨窗与SurfaceMIP对比,差异有显著性(P<0.05),VR与SurfaceMIP对比,差异无显著性(P>0.05)。结论64排螺旋CT的VR及SurfaceMIP三维重建有利于颅骨骨折的检出,其中SurfaceMIP后处理技术检出率较高,可作为颅骨骨折诊断的常规技术。

목적탐토64배라선CT( MSCT)적삼유중건술재로골골절적응용개치。방법대2013년1~8월본원로뇌외상환자252례행MSCT소묘,소묘자료송도Philips공작참EBW진행용적재현( VR)、표면밀도투영( SurfaceMIP)처리,유2명유경험적방사과의생맹법평고축위골창、VR급SurfaceMIP삼유중건도상대골절적검출정황。결과축위골창、VR급SurfaceMIP검출골절례수분별위25례、41례급49례,축위골창여SurfaceMIP대비,차이유현저성(P<0.05),VR여SurfaceMIP대비,차이무현저성(P>0.05)。결론64배라선CT적VR급SurfaceMIP삼유중건유리우로골골절적검출,기중SurfaceMIP후처리기술검출솔교고,가작위로골골절진단적상규기술。
Objective To explore the diagnostic value of Three-dimensional reconstruction with Philips 64 multi-slice CT( MSCT) in skull fracture. Method 252 patients who were suffering from craniocerebral injuries from 2013 Janu-ary to August were performed MSCT scanning. All the raw data were send to Philips workstation EBW to reconstruct with volume render( VR) and surface maximum intensity projection( SurfaceMIP) . For all of the images were evalu-ated by 2 experienced radiologists. The detection of skull fracture were blindly evaluated by VR, SurfaceMIP as well as axial bone window. Result There were 25, 41, 49 cases of fracture were founded on axial bone window, VR and SurfaceMIP respectively. There was significantly difference of detected rate between SurfaceMIP with axial bone win-dow(P<0. 05),while there was no significantly difference between SurfaceMIP with VR(P>0. 05). Conclusion VR and SurfaceMIP with MSCT can improve the detection of skull fracture,while SurfaceMIP depict skull fracture with higher accuracy . VR and SurfaceMIP can be used as conventional technology in craniocerebral injuries.
