目的介绍一种改良式手术切口治疗腮腺肿瘤的临床方法。方法改良的腮腺切除术主要要点：采用耳屏前隐蔽的面部除皱切口及耳后延续入发髻内切口，在嚼肌筋膜（SM AS）下翻瓣，减少了术后 Frey’s综合症的发生。首先解剖面神经总干减少了周围支的损伤，暴露面神经总干后，根据肿瘤的位置，选择性解剖面神经颈面干而行腮腺部分切除术，保护腮腺的部分功能。采用蒂在上方的胸锁乳突肌填塞局部减少了术后畸形；结果应用改良术式的16例病人中，无1例出现涎瘘及面神经损伤，随访3－6个月无1例出现 Frey’s综合症，并且局部凹陷轻微，面容美观；结论改良式手术方法切口隐蔽，容易操作，不易损伤面神经周围支，术后并发症少，是一种新型的腮腺肿瘤治疗方法。

목적개소일충개량식수술절구치료시선종류적림상방법。방법개량적시선절제술주요요점：채용이병전은폐적면부제추절구급이후연속입발계내절구，재작기근막（SM AS）하번판，감소료술후 Frey’s종합증적발생。수선해부면신경총간감소료주위지적손상，폭로면신경총간후，근거종류적위치，선택성해부면신경경면간이행시선부분절제술，보호시선적부분공능。채용체재상방적흉쇄유돌기전새국부감소료술후기형；결과응용개량술식적16례병인중，무1례출현연루급면신경손상，수방3－6개월무1례출현 Frey’s종합증，병차국부요함경미，면용미관；결론개량식수술방법절구은폐，용역조작，불역손상면신경주위지，술후병발증소，시일충신형적시선종류치료방법。
Objective :To describe a clinical therapy of parotidectomy with modified technique .M ethods :The modified technique indudes using a face life incisin to avoid the scar in the submandibular region ;using Interposing flap or the SM AS to avoid the incidence of Frey ’s syn-drom ;Using facial nerve dissection begin with main trunk to avoid the ingury Of peripheral branches ;Using the technique of partial parotid-ectomy according to the location to protect the partial function of parotid gland ;using a flap with an upper pedicle take from the sfernodeido-mastoid muscle to repair the defect after parotidecfomy .Result :sixteen patients were operated by using the modified technique ,all cases showed good aesthetic results with no obvious deformity of parotid gland and ingury of facial nerve .Observed 3 months to 6 months ,all pa-tients’ incisions looked well .Conclusion :The incision of modified technique of parotidectomy can not be found easily .It can be operated easi-ly and protects the peripheral branches of facial nerve .The modified technique is a new method of parotidectormy with less compliation .
