目的探讨人类白细胞抗原-G（ HLA-G）与胎膜早破（ PROM）关系及临床意义。方法随机选取2011年7月至2013年7月临产前行剖宫产分娩的初产妇，检测肘静脉血、羊水及胎膜，分为正常完好胎膜组、足月胎膜早破组和足月前胎膜早破组。应用双抗体夹心酶联免疫吸附实验测定组对象血清、羊水中HLA-G的浓度，比较各组间的相关性。结果正常完好胎膜产妇体内的HLA-G水平以及表达率均比胎膜早破产妇高，并且无感染的正常妊娠产妇胎膜的HLA-G表达率明显比亚临床感染要高，两者经检验均具显著性差异（ P <0.05）。结论 HLA-G在胎盘中的低表达可能与孕妇感染有关，而HLA-G在胎盘中的低表达可能又与胎膜早破的发生有关，这对于HLA-G在PROM上的研究有着非常重要的意义。
목적탐토인류백세포항원-G（ HLA-G）여태막조파（ PROM）관계급림상의의。방법수궤선취2011년7월지2013년7월임산전행부궁산분면적초산부，검측주정맥혈、양수급태막，분위정상완호태막조、족월태막조파조화족월전태막조파조。응용쌍항체협심매련면역흡부실험측정조대상혈청、양수중HLA-G적농도，비교각조간적상관성。결과정상완호태막산부체내적HLA-G수평이급표체솔균비태막조파산부고，병차무감염적정상임신산부태막적HLA-G표체솔명현비아림상감염요고，량자경검험균구현저성차이（ P <0.05）。결론 HLA-G재태반중적저표체가능여잉부감염유관，이HLA-G재태반중적저표체가능우여태막조파적발생유관，저대우HLA-G재PROM상적연구유착비상중요적의의。
Objective To study the relationship of HLA-G and premature rupture of membranes,and clinical significance. Methods Randomly selecting patients in our hospital in 2011 July to 2013 July. All these patients would receive cesarean delivery. The elbow venous blood, amniotic fluid and amniotic membranes grouping before delivery were collected. Antibody sandwich enzyme-linked immunosorbent was used to determine the amniotic fluid serum concentrations of HLA-G. The correlation between each group was compared. Results HLA-G level of nor-mal intact amniotic membranes of maternal body and expression rate are statistically better than the premature rupture of membranes. And HLA-G expression in normal pregnancy,maternal fetal membranes and non-infected was significantly higher than subclinical infection. All these com-parisons showed significant difference( P <0. 05). Conclusion Low expression of HLA-G in the placenta may be associated with infection in pregnant women. Low expression of HLA-G in the placenta may be related to the occurrence of premature rupture of membranes,which has a ver-y important for the study of HLA-G on the PROM significance.
