Cancer Pain%Oxycodone%Titration
目的：探讨羟考酮缓释片用于癌痛患者止痛滴定的可行性。方法：48例中重度疼痛的阿片未耐受癌症患者，中位年龄74.5岁，分为 A（中度疼痛）、B（重度疼痛7~8分）、C（重度疼痛9~10分）三组，盐酸羟考酮缓释片初始剂量分别为10 mg、10 mg、20 mg 联合短效吗啡针剂5 mg~10 mg，根据患者疼痛情况及爆发痛治疗情况于12 h 和24 h 调整奥施康定的剂量。评估24 h 滴定完成率（疼痛评分3分以下）以及1 h、12 h、24 h 疼痛明显缓解率、不良反应及生活质量的变化。结果：24 h 评估时95.8%的患者完成滴定，疼痛评分降到3分以下。1 h、12 h、24 h 疼痛明显缓解率分别为66.7%、77.1%和95.8%。预防治疗可显著降低恶心、呕吐、便秘不良反应。止痛治疗后睡眠质量明显改善。结论：奥施康定用于阿片未耐受中重度癌痛患者滴定，有效、简便、快速。
目的：探討羥攷酮緩釋片用于癌痛患者止痛滴定的可行性。方法：48例中重度疼痛的阿片未耐受癌癥患者，中位年齡74.5歲，分為 A（中度疼痛）、B（重度疼痛7~8分）、C（重度疼痛9~10分）三組，鹽痠羥攷酮緩釋片初始劑量分彆為10 mg、10 mg、20 mg 聯閤短效嗎啡針劑5 mg~10 mg，根據患者疼痛情況及爆髮痛治療情況于12 h 和24 h 調整奧施康定的劑量。評估24 h 滴定完成率（疼痛評分3分以下）以及1 h、12 h、24 h 疼痛明顯緩解率、不良反應及生活質量的變化。結果：24 h 評估時95.8%的患者完成滴定，疼痛評分降到3分以下。1 h、12 h、24 h 疼痛明顯緩解率分彆為66.7%、77.1%和95.8%。預防治療可顯著降低噁心、嘔吐、便祕不良反應。止痛治療後睡眠質量明顯改善。結論：奧施康定用于阿片未耐受中重度癌痛患者滴定，有效、簡便、快速。
목적：탐토간고동완석편용우암통환자지통적정적가행성。방법：48례중중도동통적아편미내수암증환자，중위년령74.5세，분위 A（중도동통）、B（중도동통7~8분）、C（중도동통9~10분）삼조，염산간고동완석편초시제량분별위10 mg、10 mg、20 mg 연합단효마배침제5 mg~10 mg，근거환자동통정황급폭발통치료정황우12 h 화24 h 조정오시강정적제량。평고24 h 적정완성솔（동통평분3분이하）이급1 h、12 h、24 h 동통명현완해솔、불량반응급생활질량적변화。결과：24 h 평고시95.8%적환자완성적정，동통평분강도3분이하。1 h、12 h、24 h 동통명현완해솔분별위66.7%、77.1%화95.8%。예방치료가현저강저악심、구토、편비불량반응。지통치료후수면질량명현개선。결론：오시강정용우아편미내수중중도암통환자적정，유효、간편、쾌속。
Objective:To explore the efficacy and safety of oxycodone hydrochloride extended-release tablets in the titration of cancer pain in opioid intolerance elderly patients.Method:48 patients with moderate or severe cancer pain(median age 74.5)were divided into three groups:group A with moderate pain,group B with severe pain score 7-8,group C with severe pain score 9-10.The initial dose of group A,B,C were 10 mg,10 mg,20 mg,which combined with short-acting morphine sulfate were 5 mg-10 mg.During the course of treatment,short-acting morphine sulfate was given according breakthrough pain and oxycodone dosage was adjusted at 12 hour and 24 hour.Data are assessed according to overall titration-finished rate(pain score 3 point below)at 24 hour,pain significant remission at 1 hour,12 hour and 24 hour,adverse events and quality of life changes.Result:The overall titration-finished rate at 24 hour was 95.8%.The pain significant remission at 1 hour,12 hour and 24 hour were 66.7%、77.1% and 95.8% respectively.Oxycodone side effects of nausea, vomiting and constipation were decreased significantly by prophylactic treatment.Quality of sleep was improved significantly after treatment.Conclusion:Oxycodone is effective,convenient,and rapid in titration of moderate and severe cancer pain in opioid intolerance elderly patients.
