目的:分析冠心病患者心率变异性( HRV)各项指标改变差异的临床意义。方法以动态心电图中的时域指标SDNN(连续24 h内正常R-R间期的标准差)、SDANN(全程记录中每5 min正常R-R间期平均值的标准差)、RMSSD(相邻正常R-R间期差值的均方根)、PNN50(相邻R-R间期差值>50 ms百分比)等对200例健康者、501例冠心病患者的心率变异性进行对比研究,并比较2组患者心电图的频域指标总功率( TP)、低频功率( LF)、高频功率( HF)。结果冠心病患者的24 h HRV时域指标明显低于健康对照,其频域指标LF、HF也显著下降。结论冠心病患者HRV降低,提示支配心脏的自主神经功能受损害。

목적:분석관심병환자심솔변이성( HRV)각항지표개변차이적림상의의。방법이동태심전도중적시역지표SDNN(련속24 h내정상R-R간기적표준차)、SDANN(전정기록중매5 min정상R-R간기평균치적표준차)、RMSSD(상린정상R-R간기차치적균방근)、PNN50(상린R-R간기차치>50 ms백분비)등대200례건강자、501례관심병환자적심솔변이성진행대비연구,병비교2조환자심전도적빈역지표총공솔( TP)、저빈공솔( LF)、고빈공솔( HF)。결과관심병환자적24 h HRV시역지표명현저우건강대조,기빈역지표LF、HF야현저하강。결론관심병환자HRV강저,제시지배심장적자주신경공능수손해。
Objective To analyze the clinical significance of index change differences in coronary heart disease patients with heart rate variability ( HRV) .Methods 200 normal controls and 501 cases of patients with coronary heart disease were selected and their HRV was comparatively analyzed based on the time domain SDNN ( standard deviation between normal R-R within continuous 24h) of dynamic electrocardiogram indexes ,SDANN ( full record of every 5 min average standard deviation between normal R-R),RMSSD(phase difference between adjacent normal R-R value of root mean square),and PNN50(per-centage of phase difference between adjacent R-R>50 ms) .The two groups'frequency domain indexes of electrocardiogram were compared in terms of total power (TP),low-frequency(LF)power and high frequency(HF) power.Results The 24h HRV time domain index of patients with coronary heart disease was significantly lower than that of the normal controls ;the fre-quency domain indexes HF and LF dropped significantly .Conclusion The reduced HRV in coronary heart disease patients indicates the damage of cardiac autonomic nervous functions .
