目的:研究早期目标导向治疗(EGDT)和山莨菪碱对重度脓毒症患者预后的影响。方法:将2010年10月至2012年10月收住 ICU 的18岁以上重度脓毒症患者63例随机分配为治疗(EGDT 和山莨菪碱)组和常规治疗对照组,比较两组24 h 乳酸清除率,序贯器官衰竭估计(SOFA)评分的变化及血管活性药物使用时间、28 d机械通气时间、ICU停留时间和28 d病死率。结果:治疗组入选后24 h乳酸清除率明显高于对照组,3d和7 d SOFA评分均明显低于对照组,治疗组血管活性药物使用时间、28 d内机械通气时间、ICU停留时间和28 d病死率均明显少于对照组,P均<0.05。结论:早期目标导向治疗和山莨菪碱可明显改善重度脓毒症患者的预后。

목적:연구조기목표도향치료(EGDT)화산랑탕감대중도농독증환자예후적영향。방법:장2010년10월지2012년10월수주 ICU 적18세이상중도농독증환자63례수궤분배위치료(EGDT 화산랑탕감)조화상규치료대조조,비교량조24 h 유산청제솔,서관기관쇠갈고계(SOFA)평분적변화급혈관활성약물사용시간、28 d궤계통기시간、ICU정류시간화28 d병사솔。결과:치료조입선후24 h유산청제솔명현고우대조조,3d화7 d SOFA평분균명현저우대조조,치료조혈관활성약물사용시간、28 d내궤계통기시간、ICU정류시간화28 d병사솔균명현소우대조조,P균<0.05。결론:조기목표도향치료화산랑탕감가명현개선중도농독증환자적예후。
Objective: To study the effect of early goal-directed therapy (EGDT) combined with Anisodamine on prognosis of patients with severe sepsis. Methods: 63 Patients with severe sepsis with 18 years older were randomly divided into treatment group and control group treated by early goal-directed therapy combined with Anisodamine and routine at the ICU from Oct 2010 to Oct 2012. The 24-hour lactate clearance, the SOFA score, the vasoactive agent application times, the mechanical ventilation times in 28 days, the length of ICU stay and the 28 days mortality were compared between the two groups. Results: Compared with the control group, 24-hour lactate clearance was significantly higher, the SOFA score at 3 days and 7 days were significantly lower, the vasoactive agent application times, the mechanical ventilation times in 28 days, the length of ICU stay and 28-day mortality rate were significantly lower, all P < 0.05. Conclusion: Application of EGDT combined with Anisodamine could improve the prognosis of patients with severe sepsis.
