目的:探讨颅脑损伤后高血糖与预后的关系。方法:将我院2010年1月~2010年6月收治的100例颅脑外伤患者按格拉斯哥昏迷评分(GCS)分为GCS3~8分组26例,9~12分组11例及13~15分组63例,测定入院次日空腹血糖值。结果:血糖水平与GCS 评分有明显关系,GCS评分13~15分组血糖为(6.59±0.33)mmol/l,9~12分组血糖为(7.18±0.45)mmol/l,3~8分组血糖为(13.96±1.61)mmol/l,GCS 评分越低,血糖水平则越高(P<0.05),而血糖水平越高,病死率越高(P<0.05)。结论:颅脑损伤后血糖值可作为判断颅脑外伤严重程度的指标,控制高血糖以改善颅脑损伤患者的预后。

목적:탐토로뇌손상후고혈당여예후적관계。방법:장아원2010년1월~2010년6월수치적100례로뇌외상환자안격랍사가혼미평분(GCS)분위GCS3~8분조26례,9~12분조11례급13~15분조63례,측정입원차일공복혈당치。결과:혈당수평여GCS 평분유명현관계,GCS평분13~15분조혈당위(6.59±0.33)mmol/l,9~12분조혈당위(7.18±0.45)mmol/l,3~8분조혈당위(13.96±1.61)mmol/l,GCS 평분월저,혈당수평칙월고(P<0.05),이혈당수평월고,병사솔월고(P<0.05)。결론:로뇌손상후혈당치가작위판단로뇌외상엄중정도적지표,공제고혈당이개선로뇌손상환자적예후。
Objective: To explore the relationship of hyperglycemia and prognosis of craniocerebral injury. Methods:From Jan 2010 to Jun 2010,100 patients with traumatic brain injury were divided into three groups according to the Glasgow coma score (GCS),“GCS”3~8 group,“GCS”9~12 group,and “GCS”13~15 group. The fasting blood glucose was examined at the next day among the three groups. Results:Blood glucose levels and GCS score had obviously relationship. The glucose was (6.59±0.33)mmol/l, (7.18±0.45)mmol/l and (13.96±1.61)mmol/l in GCS score 13 to 15 group,9 to 12 group and 3 to 8 group respectively. The lower the GCS score,blood glucose level was higher (P<0.05),Similarly,the higher the blood glucose level was higher mortality rate (P<0.05). Conclusions:After craniocerebral injury,the blood glucose is an index of the injury degree. We should try to control the high blood glucose to improve the prognosis of patient with craniocerebral injury.
