目的:探讨Th1/Th2亚群在DVT发病中的变化规律及中药复方消栓通脉颗粒调节Th1/Th2亚群平衡的作用机制。方法:60例急性DVT患者分为中西医结合组(结合组)和西药组,并设30例健康查体者为正常组,采用流式细胞术及ELI-SA法观察DVT患者外周血Th1、Th2细胞比例变化及血浆中TNF-α、IFN-γ、IL-4和IL-10水平。结果:消栓通脉颗粒作用后,结合组TNF-α、IFN-γ水平低于西药组(P<0.05);结合组IL-4和IL-10水平高于西药组(P<0.05);结合组治疗后肢围改善优于西药组(P<0.05)。结论:消栓通脉颗粒可以抑制TNF-α、IFN-γ转录,促进IL-4、IL-10表达,升高Th2亚群比例,优势诱导T细胞向Th2亚群偏移,减轻炎症反应,保护血管内皮细胞,促进血栓再通,改善肢体肿胀。

목적:탐토Th1/Th2아군재DVT발병중적변화규률급중약복방소전통맥과립조절Th1/Th2아군평형적작용궤제。방법:60례급성DVT환자분위중서의결합조(결합조)화서약조,병설30례건강사체자위정상조,채용류식세포술급ELI-SA법관찰DVT환자외주혈Th1、Th2세포비례변화급혈장중TNF-α、IFN-γ、IL-4화IL-10수평。결과:소전통맥과립작용후,결합조TNF-α、IFN-γ수평저우서약조(P<0.05);결합조IL-4화IL-10수평고우서약조(P<0.05);결합조치료후지위개선우우서약조(P<0.05)。결론:소전통맥과립가이억제TNF-α、IFN-γ전록,촉진IL-4、IL-10표체,승고Th2아군비례,우세유도T세포향Th2아군편이,감경염증반응,보호혈관내피세포,촉진혈전재통,개선지체종창。
Objective To discuss the changing rule of Th1/Th2 subsets in deep vein thrombasis (DVT) and the mechanism of Chinese medicine compound Xiao Shuan Tong Mai(消栓通脉)Granules in regulating the bal-ance of Th1/Th2 subsets. Methods Sixty patients with acute DVT were divided into 2 groups;integrated tradi-tional Chinese and western medicine group ITCWMG and western medicine group WMG, 30 normal persons were selected to be in normal group. Flow cytometry and ELISA were performed in order to observe the frequen-cy change of Th1 and Th2 cells in peripheral blood and the level of TNF-α, IFN-γ, IL-4, IL-10 in plasma. Results After using Xiao Shuan Tong Mai Granules, the TNF-α and IFN-γ levels in WMG were higher than those in ITCWMG; the levels of IL-4 and IL-10 in ITCWMG were higher than those in WMG(P<0.05); the improvement of circumference was better in ITCWMG than that in WMG after treatment. Conclusion Xiao Shuan Tong Mai Granules can inhibit TNF-α and IFN-γ transcription, enhance IL-4 and IL-10 expression, and increase the proportion of Th2 subsets. It also can predominantly induce T cells response bias to Th2 sub-sets,reduce the inflammatory reaction,protect vascular endothelial cells. Thrombus recanalization can be accelerat-ed and the limb swelling will be reduced.
