　　目的探讨脓毒症患者血清甲状腺激素水平变化后预后关系。方法对117例脓毒症患者总T3（total T3,TT3）、游离T3（free T 3， FT3），总T4（total T4,TT4）、游离T4（free T4,rT4），垂体促甲状腺激素（thyroid-stimulating hormone,TSH）浓度水平与APACHE Ⅱ评分和死亡病例的相关性进行分析。结果脓毒症患者血清TT3、FT3、TT4、FT4均低于正常值， APACHE Ⅱ≤19分组与APACHE Ⅱ≥20分组比差异有统计学意义（P＜0.05），死亡组与生存组比差异有统计学意义（P＜0.05），TSH无明显改变。结论脓毒症患者血清甲状腺减激素水平与脓毒症病情危重程度呈负相关，病情越严重，血清TT3、FT3、TT4、FT4越低，死亡风险越大，可以作为脓毒症患者预后的临床参考指标。
　　목적탐토농독증환자혈청갑상선격소수평변화후예후관계。방법대117례농독증환자총T3（total T3,TT3）、유리T3（free T 3， FT3），총T4（total T4,TT4）、유리T4（free T4,rT4），수체촉갑상선격소（thyroid-stimulating hormone,TSH）농도수평여APACHE Ⅱ평분화사망병례적상관성진행분석。결과농독증환자혈청TT3、FT3、TT4、FT4균저우정상치， APACHE Ⅱ≤19분조여APACHE Ⅱ≥20분조비차이유통계학의의（P＜0.05），사망조여생존조비차이유통계학의의（P＜0.05），TSH무명현개변。결론농독증환자혈청갑상선감격소수평여농독증병정위중정도정부상관，병정월엄중，혈청TT3、FT3、TT4、FT4월저，사망풍험월대，가이작위농독증환자예후적림상삼고지표。
Objective?Ananoss of exploring sepsis in patiente with thyroid homone levels. Methods 117 cases of sepsis in patients with total T3(total T3, TT3)free T3(free of T3 and FT3),total T4(total T4 in RT4),pituitary thyroid stimulating homone(thyroid-stimulating homone of TSH)levels with APACHE II score and the deaths of the correlation analysis. Results Patients with sepsis serum TT3 and FT3, TT4, FT4 were lower then normal values of APACHE II ≤19 group of APACHE II≥20 group than the difference was statiatfically significant (P<0.05), the death group and survival group than in the difference was statistically significant(P<0.05), TSH no significant change. Conclusion Sepsis in patients with serum thgroid less hormone levels with sepsis severity of illness was negatively correlated with the more serious the illness,the serum of TT3 and FT3 , TT4 and FT4 lower as a clinical reference of the prognosis of patients with sepsis indicators.
