目的研究伽玛刀治疗原发性三叉神经痛的疗效,评价治疗剂量与疗效的关系。方法对2002年7月至2010年8月间124例患者进行伽玛刀治疗。选择靶点为三叉神经根近端,伽玛刀治疗处方剂量为70~90Gy。根据不同的照射剂量随机分为<80 Gy和>80 Gy两组。治疗后定期随访,进行疗效分析。结果随访124例,随访21~69个月(平均32个月)。疼痛完全消失62例(50%),疼痛基本消失31例(25%),疼痛缓解23例(18.5%),无效8例(6.5%),有效率93.5%。治疗中心剂量<80 Gy和>80 Gy两组在显效率方面没有显著差异(P>0.01)。结论伽玛刀治疗原发性三叉神经痛安全有效,治疗剂量考虑75~80Gy为最佳。

목적연구가마도치료원발성삼차신경통적료효,평개치료제량여료효적관계。방법대2002년7월지2010년8월간124례환자진행가마도치료。선택파점위삼차신경근근단,가마도치료처방제량위70~90Gy。근거불동적조사제량수궤분위<80 Gy화>80 Gy량조。치료후정기수방,진행료효분석。결과수방124례,수방21~69개월(평균32개월)。동통완전소실62례(50%),동통기본소실31례(25%),동통완해23례(18.5%),무효8례(6.5%),유효솔93.5%。치료중심제량<80 Gy화>80 Gy량조재현효솔방면몰유현저차이(P>0.01)。결론가마도치료원발성삼차신경통안전유효,치료제량고필75~80Gy위최가。
Objective To Study and evaluate the effects of Gamma knife to treat ingprimary trigeminal neuralgia. Methods Between July 2002 and August 2010, 124 patients with primary trigeminal neuralgia were treated with Gamma Knife. the chosen target was the root of trigeminal nerve just anterior to pons, The prescribed doses of the treatments were 70~90Gy.The patients were randomly divided into tow groups(<80 Gy和>80 Gy).After treatment, the patient were followed up for long time. Results 124 patients were followed up for 21 to 69 months with mean term of 32 months. The pain completely disappeared in 62 cases (50%), The pain basically disappeared in 31 cases(25%), the pain relief occurred in 23 cases (18.5%),and in 8 patient (6.5%) the pain was unchanged,efficiency 93.5%. There wasn't significant difference in pain relief between the tow groups(P>0.01). Conclusion Treatment of primary trigeminal neuralgia with Gamma knife is effective and safe methods. 75~80Gy radiation dose should be as most suitable.
