　　目的探讨动态监测血清降钙素原(PCT)水平与危重症患者预后的相关性。方法采用前瞻性研究，对83例危重症患者在急诊入院时及入院后1、2、3、5、7d分别测定血清PCT水平，并记录其急性生理学与慢性健康状况Ⅱ(APACHEⅡ)评分、序贯器官衰竭估计(SOFA)评分、院内感染率；并对其预后进行评定。结果死亡组患者的PCT值在2、3、5、7d均明显高于生存组，差异具有统计学意义(P<0.05-0.01)；两组在A-PACHEⅡ评分、SOFA评分及院内感染率等比较，差异也具有统计学意义（P<0.05-0.01)。结论危重症患者血清PCT水平可作为反映其病情严重程度及预后的一个重要指标。
　　목적탐토동태감측혈청강개소원(PCT)수평여위중증환자예후적상관성。방법채용전첨성연구，대83례위중증환자재급진입원시급입원후1、2、3、5、7d분별측정혈청PCT수평，병기록기급성생이학여만성건강상황Ⅱ(APACHEⅡ)평분、서관기관쇠갈고계(SOFA)평분、원내감염솔；병대기예후진행평정。결과사망조환자적PCT치재2、3、5、7d균명현고우생존조，차이구유통계학의의(P<0.05-0.01)；량조재A-PACHEⅡ평분、SOFA평분급원내감염솔등비교，차이야구유통계학의의（P<0.05-0.01)。결론위중증환자혈청PCT수평가작위반영기병정엄중정도급예후적일개중요지표。
Objective To evaluate the performance of procalcitonin (PCT) as the marker of prediction of prog-nosis of criticaly il patients. Methods A total of 83 criticaly il patients in ICU were selected.The level of PCT in serum was measured with in the emergency department of the hospital and the hospital after 1, 2, 3, 5, 7 d .Al patients were scored by acute physiology and chronic health evaluationⅡ(A-PACHEⅡ) and sequential organ failure assessment (SOFA)and rate of in-hospital infection . And to evaluate its prognosis. Results Patients who died at a PCT value of 2, 3, ,5,7d were significantly higher than that in the survival group, with significant difference (P<0.05-0.01);Comparison between the twogroups in A-PACHE I score, SOFA score and the hospital infection rate, also has significant difference (P<0.05-0.01).Conclusion Serum level of PCT was an important index for the extent of severity and prognosis in criticaly il patients.
