　　目的比较腹腔镜手术与传统开腹手术对小儿急性坏疽性阑尾炎的治疗效果。方法2009年5月-2012年3月来九江市都昌县妇幼保健院就诊的105例急性坏疽性阑尾炎患儿，使用随机数表将以上患者随机分成腹腔镜组50例和传统开腹组55例。比较两组患儿年龄、手术时间、住院时间、术后进食时间和术后并发症情况。结果腹腔镜组和传统开腹手术组手术时间分别为（58±8）min和（62±10）min，P>0.05，无显著性差异，而腹腔镜手术组患者术后进食时间（1.5±0.5）d,比开腹手术组(2.5±1.2)d早，P<0.05，腹腔镜组的住院时间比传统开腹组短，且并发症少，P<0.05。结论腹腔镜手术治疗儿童急性坏疽性阑尾炎安全有效且术后并发症少。
　　목적비교복강경수술여전통개복수술대소인급성배저성란미염적치료효과。방법2009년5월-2012년3월래구강시도창현부유보건원취진적105례급성배저성란미염환인，사용수궤수표장이상환자수궤분성복강경조50례화전통개복조55례。비교량조환인년령、수술시간、주원시간、술후진식시간화술후병발증정황。결과복강경조화전통개복수술조수술시간분별위（58±8）min화（62±10）min，P>0.05，무현저성차이，이복강경수술조환자술후진식시간（1.5±0.5）d,비개복수술조(2.5±1.2)d조，P<0.05，복강경조적주원시간비전통개복조단，차병발증소，P<0.05。결론복강경수술치료인동급성배저성란미염안전유효차술후병발증소。
Objective To compare the effect of laparoscopic surgery and conventional open surgery on the treatment of children with acute gangrenous appendicitis. Methods Laparoscopic group and traditional open surgery group operative time was (58 ± 8) min and (62 ± 10) min,P>0.05, no signiifcant differences in the patients after laparoscopic surgery group feeding time (1.5 ± 0.5 ) d, than the open surgery group (2.5 ± 1.2) d early, P<0.05, laparoscopic group of hospital stay shorter than conventional laparotomy group, and fewer complications, (P<0.05). Results Laparoscopic and traditional open surgery was no signiifcant difference in operative time. And laparoscopic surgical patients with postoperative feeding time was earlier than the open surgery group patients, shorter hospital stay, and complication rate, with a signiifcant difference (P<0.05).Conclusion Laparoscopic surgery in children with acute gangrenous appendicitis is safe and effective and has fewer complications.
