目的：观察硝苯地平控释片（拜新同）对老年原发性高血压患者血压变异性（ BPV）及血清胱抑素C （ Cys-C）水平的影响。方法选取医院老年原发性高压患者62例，随机分为对照组30例及治疗组32例，治疗组予硝苯地平控释片治疗；对照组给予安慰剂作为对照。用药后第2周末做动态血压监测及检测血清胱抑素C的含量，观察BPV及其对早期肾功能的影响。结果治疗组的收缩压变异性和舒张压变异性均较对照组有明显下降，血清Cys-C含量明显降低，差异均有统计学意义（ P＜0.05）。24h血压变异性与血清Cys-C含量呈显著正相关，血压变异性与高血压性肾脏损害密切相关。结论硝苯地平控释片可有效降低老年原发性高血压患者的血压变异性，同时降低血清Cys-C水平，对肾脏具有一定的保护作用。
목적：관찰초분지평공석편（배신동）대노년원발성고혈압환자혈압변이성（ BPV）급혈청광억소C （ Cys-C）수평적영향。방법선취의원노년원발성고압환자62례，수궤분위대조조30례급치료조32례，치료조여초분지평공석편치료；대조조급여안위제작위대조。용약후제2주말주동태혈압감측급검측혈청광억소C적함량，관찰BPV급기대조기신공능적영향。결과치료조적수축압변이성화서장압변이성균교대조조유명현하강，혈청Cys-C함량명현강저，차이균유통계학의의（ P＜0.05）。24h혈압변이성여혈청Cys-C함량정현저정상관，혈압변이성여고혈압성신장손해밀절상관。결론초분지평공석편가유효강저노년원발성고혈압환자적혈압변이성，동시강저혈청Cys-C수평，대신장구유일정적보호작용。
Objective To observe the effects of nifedipine controlled release tablet on blood pressure variability ( BPV)and the serum levels of Cys-C of eldly patients with primary hypertension. Methods 62 cases of eldly patients with primary hypertension were randomly divided into control group(30 cases)and treatment group(32 cases). The treatment group was given nifedipine controlled release tablet for treatment,while the control group was given placebo. After two weeks, the BPV was measured by blood pressure monitoring,and the serum levels of Cys-C were examined. Observe the BPV and its influence on the early renal function. Results Compared with control group,the blood variability of systolic and diastolic blood pressure was decreased significantly in treatment group,and the serum levels of Cys-C was decreased,the differences were statistically significant(P<0. 05). The indexe of BPV was significant positive correlation with the levels of Cys C. The Blood pressure variability of hypertensive patients was correlated to early hypertensive renal damage. Conclusion Nifedipine controlled release tablet can decrease the variability of blood pressure of eldly patients with primary hypertension,and also the levels of Cys-C,which may have a protective effect on kidney.
