目的：探讨分析复杂性的肾结石患者行经皮肾镜下应用第四代EMS碎石清石系统的治疗方法。方法对该院2008年10月-2013年6月收治的179例复杂性的肾结石患者经皮肾镜下运用第四代的EMS碎石清石系统进行治疗的资料进行回顾性分析，得出分析结果。结果这179例患者的手术过程都是非常顺利的，运用单通道手术方法取石的患者有171例，运用2~3个通道进行取石的患者有8例；一次取石的患者有174例，进行第二次取石的患者有5例，手术的时间范围是30分钟到180 min，平均时间是（89±18） min。留置肾造瘘管的平均时间是5 d，在手术之后的平均住院时间是7 d。在这179例患者中间有166例患者的肾结石得到完全清除；结石的清除率是92.7%。结论经皮肾镜下运用第4代EMS碎石清石系统治疗复杂性的肾结石患者，临床治疗效果非常好，值得推广和应用。
목적：탐토분석복잡성적신결석환자행경피신경하응용제사대EMS쇄석청석계통적치료방법。방법대해원2008년10월-2013년6월수치적179례복잡성적신결석환자경피신경하운용제사대적EMS쇄석청석계통진행치료적자료진행회고성분석，득출분석결과。결과저179례환자적수술과정도시비상순리적，운용단통도수술방법취석적환자유171례，운용2~3개통도진행취석적환자유8례；일차취석적환자유174례，진행제이차취석적환자유5례，수술적시간범위시30분종도180 min，평균시간시（89±18） min。류치신조루관적평균시간시5 d，재수술지후적평균주원시간시7 d。재저179례환자중간유166례환자적신결석득도완전청제；결석적청제솔시92.7%。결론경피신경하운용제4대EMS쇄석청석계통치료복잡성적신결석환자，림상치료효과비상호，치득추엄화응용。
Objective The treatment method of percutaneous nephrolithotomy using fourth-generation EMS rubble stone clearance system for patients with complex kidney stones was analyzed and summarized. Methods The treatment data of 179 cases of pa-tients with complex kidney stones treated by percutaneous nephrolithotomy using the fourth generation of EMS rubble stone clear-ance system in our hospital from October, 2008 to June, 2013 were retrospectively analyzed and the result was obtained. Results The surgical procedures of 179 cases of patients were very successful. 171 cases adopted single channel operation method to take out kidney stones, 8 cases used two or three channel operation method. 174 cases took out the kidney stones for one time, 5 cases removed the stones for the second time. The operation time range was from 30 min to 180 min, the average time was 89 ±18 min. The average time of indwelling renal fistula was 5 days, average hospitalization time after operation was 7 days. Of the 179 cases, the kidney stones of 166 cases were removed completely; the stone clearance rate was 92.7%. Conclusion The clinical effect of percutaneous nephrolithotomy using fourth-generation EMS rubble stone clearance system on patients with complex kidney stones is very good, which is worthy of promotion and application.
