目的:研究表明有氧运动可缓解乳腺癌患者因化疗引起的癌因性疲乏( CRF),但仍有部分研究存在争议性结论,本研究旨在探讨有氧运动对乳腺癌化疗患者 CRF 的干预疗效。方法计算机检索 Pubmed、Embase、CISCOM、CINAHL、谷歌学术、CBM 和中国知网等中英文数据库。检索范围从建库至2013-09-01,采用 STATA 12.0软件进行 Meta 分析,计算标准均方差(SMD)及其95%可信区间(95% CI)评价两者关联性。结果本研究共计纳入了12项病例对照研究共1014例乳腺癌化疗患者,其中包括522例有氧运动患者(试验组)和492例对照患者(对照组)。Meta 分析的结果表明:试验组的 Piper 疲乏量表( RPFS)评分低于对照组,差异有统计学意义〔 SMD =-0.82,95% CI(-1.04,-0.60),P ﹤0.001〕;然而,癌症治疗功能评估疲乏量表(FACIT - F)评分在两组间差异无统计学意义〔SMD =0.09,95% CI(-0.07,0.25),P =0.224〕。根据不同种族进行亚组分析发现,两组 RPFS 和FACIT - F 评分在亚洲人群中差异有统计学意义〔 RPFS:SMD =-1.08,95% CI(-1.35,-0.82),P ﹤0.001;FACIT - F:SMD =1.20,95% CI(0.70,1.71),P ﹤0.001〕,但是在欧美人群中差异无统计学意义(P ﹥0.05)。结论有氧运动可以有效改善乳腺癌患者化疗引起的CRF,尤其在亚洲人群中表现更为明显。

Objective Studies show that aerobic exercise may receive cancer - related fatigue of breast cancer patients undergoing chemotherapy,but some with controversial results. This meta - analysis is aimed to explore the intervention effects of aerobic exercise on CRF in breast cancer patients receiving chemotherapy. Methods A literature search of PubMed,Embase, CISCOM,CINAHL,Google Scholar,CNKI and CBM databases was conducted on articles published before September 1st, 2013. STATA 12. 0 software was used for the meta - analysis. Crude standardized mean difference(SMD)and its 95% confidence interval(CI)were calculated,and according to the results the association of the two was assessed. Results Twelve cases were brought into the comparative studies with a total of 1 014 breast cancer patients receiving chemotherapy,including 522 patients in the aerobic exercise group(the intervention group)and 492 patients in the usual care group(the control group). The meta - a-nalysis results revealed that the RPFS scores of breast cancer patients in the intervention group were significantly lower than those in the control group〔SMD = - 0. 82,95% CI( - 1. 04, - 0. 60),P ﹤ 0. 001〕. However,there was no significant difference in FACIT - F scores between the intervention and control groups〔SMD = 0. 09,95% CI( - 0. 07,0. 25),P = 0. 224〕. Sub-group analysis by ethnicity indicated that there were significant differences in RPFS and FACIT - F scores between the intervention and control groups among Asian populations〔RPFS:SMD = - 1. 08,95% CI( - 1. 35, - 0. 82),P ﹤ 0. 001;FACIT - F:SMD = 1. 20,95% CI(0. 70,1. 71),P ﹤ 0. 001〕,but no significance among Caucasian populations(P ﹥ 0. 05). Conclu-sion The current meta - analysis indicates that aerobic exercise may improve cancer - related fatigue in breast cancer patients re-ceiving chemotherapy,especially among Asian populations.
