　　目的：探讨乌司他丁联合奥曲肽治疗急性胰腺炎的临床疗效。方法：将76例确诊为急性胰腺炎患者随机分为治疗组和对照组各38例。两组患者均给予内科常规治疗,包括禁食,胃肠减压,补充血容量,维持水电解质和酸碱平衡,营养支持,腹痛剧烈者可予哌替啶,预防性应用抗生素等对症治疗。对照组在内科常规治疗基础上加用醋酸奥曲肽针静脉滴注。治疗组在对照组治疗基础上予乌司他丁静脉滴注。两组疗程均为7d。结果：治疗组总有效率为92.11%,对照组总有效率为73.68%,两组总有效率比较,经χ2检验,差异具有统计学意义(P<0.05)。两组治疗后血浆TNFα、IL-6、IL-8水平与治疗前比较明显降低,IL-10水平与治疗前比较明显提高,组内治疗前后比较差异具有统计学意义(P<0.05,P<0.01)；治疗后,治疗组在TNFα、IL-6、IL-8、IL-10水平变化与对照组比较,均有统计学意义(P<0.05)。结论：乌司他丁联合奥曲肽治疗能够减轻急性胰腺炎患者炎症反应及细胞组织的损伤,提高临床疗效,值得推广使用。
　　목적：탐토오사타정연합오곡태치료급성이선염적림상료효。방법：장76례학진위급성이선염환자수궤분위치료조화대조조각38례。량조환자균급여내과상규치료,포괄금식,위장감압,보충혈용량,유지수전해질화산감평형,영양지지,복통극렬자가여고체정,예방성응용항생소등대증치료。대조조재내과상규치료기출상가용작산오곡태침정맥적주。치료조재대조조치료기출상여오사타정정맥적주。량조료정균위7d。결과：치료조총유효솔위92.11%,대조조총유효솔위73.68%,량조총유효솔비교,경χ2검험,차이구유통계학의의(P<0.05)。량조치료후혈장TNFα、IL-6、IL-8수평여치료전비교명현강저,IL-10수평여치료전비교명현제고,조내치료전후비교차이구유통계학의의(P<0.05,P<0.01)；치료후,치료조재TNFα、IL-6、IL-8、IL-10수평변화여대조조비교,균유통계학의의(P<0.05)。결론：오사타정연합오곡태치료능구감경급성이선염환자염증반응급세포조직적손상,제고림상료효,치득추엄사용。
Objective:Explore clinical therapeutic effect of ulinastain combined with octreotide for treating acute pancreatitis. Methods:76 confirmed cases of patients with acute pancreatitis were randomly assigned to treatment and control groups,each 38 cases. Both groups of patients were given conventional treatment of medical department, including fasting,gastric decompression,supplementary blood volume,maintenance of water electrolysis and acid-base balance,nutritional support,patients with intense abdominal pain can take pethidine,prophylactic symptomatic treatment such as prophylactic use of antibiotics.Control group was given octreotide acetate intravenous drip on the basis of conventional treatment of medical department.Treatment group was given ulinastain intravenous drip on the basis of control group treatment.Courses of treatment of two groups were both 7d.Results:Total effective rate of treatment group was 92.11%,total effective rate of control group was 73.68%,there was significant difference between total effective rates of two groups according to χ2 test(P<0.05).Plasma TNFα、IL-6、IL-8 levels of post-treatment two groups significantly reduced compared with those of pretherapy two groups,IL-10 level significantly improved compared with pretherapy,there was statistical significance between pretherapy and post-treatment in each group(P<0.05,P<0.01);compared with control group,TNFα、IL-6、IL-8、IL-10 levels changes of treatment group had statistical significance(P<0.05).Conclusion:Ulinastain combined with octreotide treatment can reduce inflammatory response and tissue injury in patients with acute pancreatitis,improve clinical therapeutic effect,worth promotion.
