目的探讨自体垂直悬吊牵引对新西兰大白兔退变椎间盘高度变化的影响。方法20只新西兰大白兔以16 G针刺损伤新西兰大白兔腰椎椎体右前侧纤维环制备椎间盘退变模型,随机分为牵引组和对照组。牵引组(n=15)行自体垂直悬吊牵引,每天固定牵引30 min,连续牵引8周。对照组(n=5)不予治疗。分别于牵引后2、4、6、8周拍摄腰椎侧位X片,计算椎间盘高度指数百分比(DHI%)。结果牵引组牵引2周、4周后均较前有显著性差异(P<0.05),治疗效果在0~4周区间呈上升趋势,4周后呈下降趋势。牵引组治疗后各个时间点DHI%均显著大于对照组(P<0.001)。结论自体垂直悬吊牵引能够延缓和抑制椎间盘退变。

목적탐토자체수직현조견인대신서란대백토퇴변추간반고도변화적영향。방법20지신서란대백토이16 G침자손상신서란대백토요추추체우전측섬유배제비추간반퇴변모형,수궤분위견인조화대조조。견인조(n=15)행자체수직현조견인,매천고정견인30 min,련속견인8주。대조조(n=5)불여치료。분별우견인후2、4、6、8주박섭요추측위X편,계산추간반고도지수백분비(DHI%)。결과견인조견인2주、4주후균교전유현저성차이(P<0.05),치료효과재0~4주구간정상승추세,4주후정하강추세。견인조치료후각개시간점DHI%균현저대우대조조(P<0.001)。결론자체수직현조견인능구연완화억제추간반퇴변。
Objective To explore the effect of gravitational traction on height change of degenerative disc of New Zealand white rabbit. Methods In order to create intervertebral disc degeneration models, the right front side of the annulus fibrosus of lumbar vertebrae of rabbits were damaged by 16-gauge needle. After confirming the success of modeling, 20 animal models were randomized into 2 groups, 15 of which received a routine gravitational traction using a model of our own design, 30 min per day, for 8 weeks, while the control group re-ceived no treatment at all. Radiographic exam were performed in the 2nd, 4th, 6th and 8th week, after intervention and the disc height index percent (DHI%). Results Compared to the baseline, DHI%increased significantly 2 and 4 weeks after gravitational traction (P<0.05), and there was no further effect as the traction continued. However, compared to the control group, an improvement of DHI%was found on all the time points in the gravitational traction group (P<0.001). Conclusion Gravitational traction may delay and inhibit intervertebral disc de-generation by increasing height of degenerated disc of New Zealand white rabbit.
